三峡库区血吸虫病疫区返乡人群日本血吸虫病危险因素分析

Analysis of the risk factors of Schistosoma japonicum infection of mobile populations returned from schistosomiasis endemic areas in the Three Gorges Reservoir areas

摘要 目的了解三峡库区返乡人群日本血吸虫感染的流行因素。方法选择三峡库区中段6个区县从血吸虫病疫区返乡的流动人员作为研究对象,用统一的调查表作个案调查,采集2~3ml静脉血检测人群血吸虫特异性抗体(IgG),进行因素分析。结果调查1940人,人群血吸虫抗体几何平均数(OD均值)为0.0176。返乡人员的文化程度(t=3.7389,P<0.05)、流动原因(t=4.0945,P<0.05)、外出后打工省份(t=3.8219,P<0.05)、疫区省类型(t=3.1574,P<0.05)、离渝前职业(t=3.0071,P<0.05)、是否常返乡(t=11.6830,P<0.05)、返乡次数(t=4.5391,P<0.05)、接触江河水的原因(t=3.0332,P<0.05)、外出后血防知识的掌握(t=2.2364,P<0.05)以及人群调查年份(t=6.6892,P<0.05)等因素与人群血吸虫感染相关。结论三峡库区返乡人群存在血吸虫感染的危险因素。建议进一步健全三峡库区血吸虫病监测体系,针对重点人群,加强监测和干预,预防和控制血吸虫病的发生和流行。 Objective To analyze the risk factors of infection with Schistosoma japonicum in the mobile populations returning from schistosomiasis endemic areas in the Three Gorges Reservoir areas(TGR).Methods The mobile populations returned from the schistosomiasis endemic areas in 6 counties of the middle part of TGR were investigated.Each subject was interviewed and information was recorded.2~3 ml venous blood was collected and analyzed by ELISA assay.Results 1 940 subjects were investigated in 2007-2009.The Geometric Mean of OD value(OD-value) of anti-schistosome antibody level was 0.017 6.Results from the multivariate stepwise regression analysis indicated that there was a significant correlation between infection and the education of the individual(t=3.738 9,P0.05),floating reasons(t=4.094 5,P0.05),the working province(t=3.821 9,P0.05),endemic type(t=3.157 4,P0.05),occupations(t=3.007 1,P0.05),the frequency of returning to their home(t=4.539 1,P0.05) and home cities(t=11.683 0,P0.05),the reasons of contacting river water(t=3.033 2,P0.05),the awareness of schistosomiasis prevention(t=2.236 4,P0.05) and the year of survey(t=6.689 2,P0.05).Conclusion The risk factors affecting schistosomiasis infection in the mobile populations returning from schistosomiasis endemic areas were identified.The monitoring system of schistosomiasis should be improved and health education should be given to the target populations in order to prevent infection of S.japonicum in the TGR.
