Prefabricated Osteocutaneous Flap for Tissue Reconstruction

摘要 目的建立一个血管束植入预构骨皮瓣的动物实验模型,以研究血管束植入后的预构骨皮瓣的形成及代谢变化。方法 40只新西兰兔按随机数字表法分为实验组和对照组,每组20只:对照组移植右髂骨块于颈单蒂任意皮瓣下方,形成骨皮瓣,实验组颈部骨皮瓣植入右耳中央动、静脉束。2组再随机分为5组,每组4只,按时间顺序在2、3、4、5、6周时观察皮瓣的成活情况、同位素扫描及组织切片。结果实验组骨皮瓣出现坏死3例,成活率85%(17/20),对照组出现坏死8例,成活率60%(12/20),植入血管束的预构骨皮瓣成活情况好;同位素扫描实验组骨皮瓣中骨组织代谢生长及血供明显优于对照组,ECT值实验组高于对照组,2组比较差异有统计学意义(P<0.01),组织学检查实验组骨组织无坏死及吸收,而对照组骨组织有坏死及吸收。结论血管束植入具有加速血运重建,保持骨组织结构稳定,无吸收,5周左右达到良好的血运重建。 Objective To establish an animal model of vessel bundle implantation prefabricated osteocutaneous flap,and to study its formation and metabolic changes after transplantation.Methods Forty New Zealand rabbits were randomly divided into experimental group and control group,with 20 rabbits in each group.The right iliac bone blocks were transplanted under the flap at cervical area to form the osteocutaneous flap in the control group,while in the experimental group,the central auricular bundle of artery and vein of the right ear was implanted into the osteocutaneous flap.The experimental group and control group were further divided into 5 groups,respectively,with 4 rabbits in each group.The vitality,isotope scanning and tissue slice of the osteocutaneous flap were observed at weeks 2,3,4,5 and 6 after implantation.Results Osteocutaneous flap necrosis occurred in 3 cases of experimental group(survival rate 85%,17/20),and 8 cases of control group(survival rate 60%,12/20).Isotope scanning displayed that implant bone-skin flap group had better results than control group in survival rate and ECT value(P0.01).Necrosis and absorption were not found in bone tissue in experimental group but were observed in control group by histological examination.Conclusion The vascular prefabricated osteocutaneous flap can provide a swift blood supply reconstruction at about 5 weeks.
