经皮微创锁定加压钢板内固定治疗老年股骨粗隆间骨折 被引量:23

Minimally invasive percutaneous plate osteosynthesis technology combined with locking compression plate internal fixation for treatment of intertrochanteric fracture in elderly patients

摘要 [目的]探讨经皮微创钢板接骨术(MIPPO)结合锁定加压钢板(LCP)内固定治疗老年股骨粗隆间骨折的手术方法及临床效果。[方法]对2008年4月~2009年10月本院收治的20例股骨粗隆间骨折患者采用MIPPO技术结合LCP进行内固定治疗。其中男9例,女11例,年龄72~92岁,平均81岁。Tronzo-Evans分型:Ⅰ型2例,Ⅱ型3例,Ⅲ型10例,Ⅳ型3例,Ⅴ型2例。[结果]20例均获随访,随访时间12~30个月,平均18个月。愈合时间11~14周,仅1例不愈合。无感染、深静脉血栓、骨折不愈合、内固定松动、断钉并发症。术后功能按Harris评分:优15例、良4例、可1例,优良率95%。[结论]MIPPO技术结合LCP内固定治疗老年股骨粗隆间骨折,手术时间短,术中创伤小,固定简约可靠,术后骨折愈合率高,并发症少,功能良好,是治疗老年股骨粗隆间骨折的良好方法。 [Objective]To investigate the surgical method and clinical effect of minimally invasive percutaneous plate osteosynthesis(MIPPO) technology combined with locking compression plate(LCP) internal fixation for treatment of intertrochanteric fracture in elderly patients. [Method]Twenty patients with intertrochanteric fracture were managed by MIPPO technology combined with LCP internal fixation.There were 9 males and 11 females with the average age of 81 years(ranged from 72 to 92 years).According to Tronzo-Evans classification:there were 2 cases for type Ⅰ,3 cases for type Ⅱ,10 cases for typeⅢ,3 cases for typeⅣ,2 cases for typeⅤ. [Result]All the patients were followed up for a mean time of 18 months(ranged from 12 to 30 months).The healing time ranged from 11 to 14 weeks.Only one patient suffered malunion.None of the wound infection and deep vein thrombosis case,none of non-union sunion and screw loosening happened in this group.According to Harris score were classified as excellent in 15,good in 4 and fair in 1,with total excellent and good rate being 95%. [Conclusion]MIPPO technology combined with locking compression plate fixation is a good method to treat intertrochanteric fracture in elderly patients for less operation time,less surgical trauma,simple and reliable fixation,high rate of bone union and excellent function.
