体外循环术中持续吸入一氧化氮对病人术后出血和肺功能的影响

摘要 目的:观察体外循环(CPB)病人术中持续吸入一氧化氮(NO)对术后出血和肺功能的影响。  方法:42例病人被分为对照组(n= 21)和一氧化氮(NO)组(n= 21)。NO组,通过肺和氧合器(CPB中)吸入浓度为40 ppm (1 ppm = 1/106)的NO。回顾性查阅所有病人的术后胸腔和纵隔引流量、术后血红蛋白含量、血小板和白细胞计数、肺泡—动脉血氧分压差及胸肺顺应性(Cdyn )等。  结果:NO组与对照组比较术后12小时和24 小时的胸腔和纵隔引流量分别减少44% 和34% ,术后4~10 小时和10~20小时的血小板计数分别增加了22% 和21% ,术后0~1小时和1~4 小时病人的肺泡—动脉血氧分压差分别减少了33% 和26% 。  结论:体外循环术中持续吸入浓度为40 ppm 的NO可减少术后出血和改善术后肺功能。 Objective:To find the effect of continuous nitric oxide (NO) inhalation on the postoperative bleeding and lung function in patients with extracorporeal bypass(CPB). Methods:Forty two patients were divided into control group and NO inhalation group(each group n =21).In NO group,40 ppm NO was inhaled through the lung and oxygenator(during CPB).The postoperative drainage,the counts of platelet and leukocyte,the P (A-a) O 2 of lung and compliance of lung and thorax(C dyn ) were studied retrospectively. Results:The postoperative drainage was reduced by 44% and 34% at 12 hours and 24 hours in NO group postoperatively.The counts of platelet was increased by 22% and 21% at 4~10 hours and 10~20 hours after operation in NO group.The P (A-a) O 2 of lung was reduced by 33% and 26% at 0~1 hours and 1~4 hours in NO group postoperatively. Conclusions:In patients with extracorporeal bypass,continuous NO inhalation can reduce postoperative bleeding and improve the postoperative lung function.
