摘要
目的为临床针对肠道外沙门菌感染的抗菌药物合理选用提供依据及预防和减少耐药菌株的产生和流行。方法分析某市级医院2002至2010年肠道外分离的61株猪霍乱沙门菌的耐药性,用MicroScan WalkAway-40全自动细菌鉴定仪及其配套阴性杆菌鉴定药敏板NC-31对分离的61株猪霍乱沙门菌进行药敏检测,用WHONET5.4软件对药敏结果进行统计分析。结果分离出的猪霍乱沙门菌中95%耐3种以上抗生素,对头孢菌素、喹诺酮类耐药率低,对头孢吡肟、头孢泊肟、头孢曲松、头孢他啶、头孢西丁、头孢噻肟耐药率为0,对头孢唑啉耐药率为2.0%,对加替沙星、左旋氧氟沙星耐药率为0,对环丙沙星耐药率为6.6%;而对氨苄西林、复方新诺明、哌拉西林、庆大霉素、氨苄西林/舒巴坦耐药率高,分别为96.7%、96.7%、95.1%、89.8%、77.0%。结论猪霍乱沙门菌对临床常用的抗菌药物有较高的耐药性,头孢菌素、氟喹诺酮类药物可作为猪霍乱沙门菌感染的首选药。临床治疗肠道外猪霍乱沙门氏菌感染时应根据药敏结果合理使用抗菌药物。
Objective To analyze the drug resistance of 61 strains extraintestinal isolated Salmonella.choleraesuis so as to guide the treatment and infection control of Salmonella.choleraesuis.Methods The strains of Salmonella.choleraesuis were isolated from clinical specimens by routine methods.The identification and drug susceptibility test of the isolates were determined by Automated Microbiology Analyzer,the data were analyzed by WHONET5.4 software.Results 95% Salmonella.choleraesuis was able to bear more than 3 kinds of common antibiotics.The drug-resistance rates of Cefepime,Cefpodoxime,Ceftriaxone,Ceftazidime,Cefoxitin,and Cefotaxime were all 0,to Gatifloxacin,Levfloxacin and Ciprofloxacin were 0,0,6.6% respectively,but to Ampicillin,Compound Sulfamethoxazole,Ciperacillin,Gentamycin,Ampicillin/Sulbactam,the drug-resistance rate was 96.7%,96.7%,95.1%,89.8%,77.0% respectively.Conclusion Salmonella.choleraesuis has high resistance rate to some common clinical used antibacterial drugs,Cephalosporins and Fluoroquinolones can be chosen in first for Salmonella.choleraesuis therapy.So antibacterial drugs should be chosen on the basis of drug resistance test in treatment of extraintestinal infection caused by Salmonella.choleraesuis.
