三种经尿道术式治疗良性前列腺增生的疗效观察 被引量:6

摘要 目的比较经尿道前列腺电气化术(TVP)、前列腺选择性光气化术(PVP)及前列腺选择性光气化术联合电气化术(PVP+TVP)3种手术方法治疗良性前列腺增生(BPH)的疗效和安全性。方法分别采用TVP、PVP、PVP+TVP 3种方式治疗BPH患者1008例。术前3组国际前列腺症状评分(IPSS)、生活质量评分(QOL)、最大尿流率(Qmax)、剩余尿量(PVR)比较差异均无统计学意义(P>0.05)。比较3组患者手术前后血清电解质和血红蛋白的变化、手术时间、术后尿管留置时间、术后并发症发生率。术后3个月随访IPSS、QOL、Qmax及PVR等。结果 TVP、PVP、PVP+TVP各组平均手术时间分别为(52.41±7.66)min、(55.97±9.24)min和(54.32±8.98)min,组间比较(P>0.05)。3组患者手术前后平均血清Na+的减少分别为(6.31±2.44)mmol/L、(0.87±0.70)mmol/L和(4.57±1.90)mmol/L,手术前后平均血红蛋白浓度的减少分别为:(15.28±5.31)g/L、(3.39±1.19)g/L和(8.78±2.97)g/L,组间比较(P<0.05);术后平均留置尿管时间为(4.68±0.90)d(、2.65±0.84)d和(3.48±0.89)d,组间比较(P<0.05);并发症发生率为7.02%、2.84%和4.49%,组间比较(P<0.05)。术后3个月随访,3组IPSS、QOL、Qmax、PVR均较术前显著改善(P<0.05)。结论 3种手术方法均有良好的疗效,与TVP相比,PVP及PVP+TVP治疗BPH,出血少、恢复快、并发症少、安全性高,临床上应根据不同的情况选择不同手术方法,以获得更好的临床疗效。
