抑郁障碍患者下丘脑-垂体-肾上腺轴调节异常对辅助性T细胞17分化发育的影响 被引量:7

Effect of dysregulation of hypothalamus-pituitary-adrenal(HPA) axis on the differentiation and development of T-helper cell 17 in depressive disorder patients

摘要 目的探讨抑郁障碍患者的下丘脑-垂体-肾上腺(hypothalamus-pituitary-adrenal,HPA)轴功能异常对辅助性T细胞17(Th17细胞)分化发育的影响。方法纳入包括单相抑郁和双相抑郁在内的首次发病未用过药的抑郁发作患者32例及正常对照25名,测定受试者的血浆促肾上腺皮质激素(adrenocorticotrophic hormone,ACTH)、血清皮质醇和细胞因子IL-17的浓度,采用逆转录-聚合酶链反应检测受试者外周血单个核细胞(peripher-al blood monouclear cells,PBMC)中糖皮质激素受体(glucocorticoid receptor,GR)和维甲酸相关核孤儿受体γt(retin-oid-related orphan receptor gammat,RORγt)mRNA的表达水平。结果抑郁障碍患者组和正常对照组的PBMC中GR mRNA的表达分别为25.14%和39.56%,患者组显著下降(Z=-2.76,P<0.01);患者组RORγt mRNA的表达、血清IL-17浓度分别为68.70%、83.83 ng/L,而对照组为24.74%、65.60 ng/L,患者组两个指标均显著升高(Z=-3.00,P<0.01;Z=-2.33,P<0.05)。患者组中GR mRNA与RORγt mRNA的表达、IL-17浓度均呈负相关(r=-0.70,P<0.01;r=-0.47,P<0.05);未见其余指标间存在相关(P>0.05)。结论 HPA轴负反馈调节异常对Th17细胞的分化发育及炎性诱导起重要作用。 Objective To investigate the potential mechanism by which abnormal HPA axis affected differentiation and development of T-helper cell 17(Th17 cell) in depressive disorder patients.Methods Thirty-two first-episode depressive disorder patients with unipolar and bipolar depression without medication and 25 healthy controls were recruited in the present study.Plasma adrenocorticotrophic hormone(ACTH),serum cortisol and IL-17 were detected.RT-PCR was used to examine the mRNA expression levels of glucocorticoid receptor(GR) and RORγt in the peripheral blood monouclear cells(PBMCs).Results Compared with in the controls,GR mRNA expression in PBMCs of depressive disorder patients was showing a significant decrease(39.56% vs 25.14%,Z=-2.76,P0.01).RORγt mRNA expression was 68.70% and 24.74% in patients and controls.In addition,the serum concentration of IL-17 were 83.83 ng/L and 65.60 ng/L in patients and controls,respectively.Compared with controls,patients had significantly higher levels of RORγt mRNA expression and serum concentration of IL-17(Z=-3.00,P0.01;Z=-2.33,P0.05).There was a negative correlation between GR mRNA and RORγt/IL-17(r=-0.70,P0.01;r=-0.47,P0.05)while there was no correlation between RORγt/IL-17 and ACTH/cortisol(P0.05).Conclusions Abnormal HPA axis plays an important role on differentiation and development as well as inflammatory induction of Th17 cells in depressive disorder.
