摘要
目的对比研究高频喷射通气机和常频普通呼吸机在硬质气管镜开放通气时不同条件下(自主呼吸完全消失及自主呼吸部分恢复时)的通气效果,进而尝试寻求解决此问题的方法。方法健康杂种犬16只,随机分为2组:高频组予高频喷射通气;常频组予常频机械通气。观察动物自主呼吸完全消失情况下及自主呼吸部分恢复后2组通气效果并比较。结果在无自主呼吸条件下,2种呼吸机均可维持氧饱和度,但均伴有较高的二氧化碳(CO2)潴留。在自主呼吸部分恢复条件下,2种呼吸机均可维持血氧饱和度,且能促进CO2排出,维持正常的p(CO2)。2组通气效果比较无显著性差异。结论在使用国产高频通气机进行全麻下硬质气管镜介入治疗时,应结合术中患者的反应及一般情况,采用小剂量肌松剂,保留患者一定程度的自主呼吸,以维持满意的氧合并减少CO2潴留。在无高频喷射呼吸机的情况下,可用常频普通呼吸机代替高频喷射呼吸机。
Objective It is to comparatively study the ventilatory effect between high frequency jet ventilation and normal frequency mechanical ventilation under different conditions(with no spontaneous breathing or with spontaneous breathing recovered partially) under rigid bronchoscope and then to explore tried to resolve this problem.Methods Sixteen healthy bastard dogs were randomly divided into two groups.High frequency group was given high frequency jet ventilation and normal frequency group was given normal frequency ventilation.The ventilatory effects of the two groups under different conditions(with no spontaneous breathing or with spontaneous breathing recovered partially) were compared.Results When dogs had no spontaneous breathing,both high frequency ventilator and normal frequency ventilator could provide enough O2 supply,but both with CO2 retention.When dogs' spontaneous breathing recovered partially,both high frequency ventilator and normal frequency ventilator could provide enough O2 supply,and could promote the release of CO2 to maintain a normal level of p(CO2).There was no significant difference on the ventilatory effect between the two groups.Conclusion When the high frequency jet ventilator made in China is used during interventional rigid bronchoscopy under intravenous anesthesia,a low dose of muscle relaxant is recommended and patients' spontaneous breathing could be maintained partially so that satisfactory ventilation effect with normal level of O2 and CO2 could be achieved.If there are no high frequency jet ventilators,normal frequency mechanical ventilators can be used instead.
