摘要
目的探讨HLA-DRB1等位基因和抗CCP抗体的相关性及两者对类风湿关节炎(Rheum atoid Arthritis,RA)关节破坏严重性的影响。方法采用SSP(序列特异性引物)多重PCR技术扩增病例组(106例)与对照组(340例)HLA-DRB1等位基因;采用酶联免疫吸附试验(ELISA)检测抗CCP抗体;同时采集RF、ESR、CRP、关节X线表现分期和病程等资料。以卡方检验分析HLA-DRB1等位基因与抗CCP抗体的相关性;以关节X线表现作为关节破坏程度的指标(为应变量),各HLA-DRB1等位基因、抗CCP抗体、病程等为自变量,以Logistic回归分析的方法分析X线表现与上述各自变量的相关性。结果 SE等位基因与抗CCP抗体的产生有显著相关性(P=0.043)。多因素分析表明以关节X线表现为应变量,HLA-DRB1*0405、病程和CRP与X线表现呈正相关。结论 HLA-DRB1等位基因对RA的影响是多方面的,即影响RA的发病易感性,也影响其病情严重性,并且与抗CCP抗体的产生显著相关。
Aim To explore the correlation between anti-CCP antibody and HLA-DRB1 alleles and explore the influence of the two factors on the arthrosis erosion of Rheumatoid Arthritis(RA).Methods Multiple sequence specific primer PCR(SSP-PCR)was used to amplify HLA-DRB1 alleles of RA(106 patients)and normal control groups(340 patients);enzyme linked immunosorbent assay(ELISA)was used to find anti-CCP antibody.The clinical data RF,ESR,CRP,X-ray of arthrosis,the function of arthrosis and disease course,and so onwere collected.Pearson Chi-square test or Fisher's Exact Test was used to test the correlation between anti-CCP antibody and HLA-DRB1 alleles;X-ray of arthrosis was used as severity index of disease(dependent variable),anti-CCP antibody,HLA-DRB1 alleles,disease course,and so on,were used as independent variables.Logistic regression was used to test the correlations between dependent variable and independent variables.Results There was significant correlation between shared epitope(SE)alleles and anti-CCP antibody(P=0.043).HLA-DRB1*0405,disease course and CRP were positively correlated with the expression of X-ray of arthrosis by logistic regression.Conclusions HLA-DRB1 alleles had several influences on RA:that is to influence the susceptibility of RA and the disease severity,furthermore the production of anti-CCP antibody.
