摘要
目的比较神经生长因子(NGF)及神经纤维在膝骨性关节炎(OA)和单纯膝关节外伤患者的表达和分布。方法应用免疫组织化学方法定性和半定量分析检测NGF在膝骨性关节炎及单纯膝关节外伤患者膝关节内下方滑膜组织的表达。用G less神经纤维染色法对滑膜组织进行神经纤维染色,观察神经纤维分布差异。结果 NGF主要在滑膜血管平滑肌层表达。实验组NGF的表达(阳性血管比率70.97%)明显高于对照组(阳性血管比率37.85%)。实验组内疼痛高分组NGF的表达高于疼痛低分组(P<0.05),膝关节功能欠佳组的表达高于膝关节功能良好组的表达(P<0.05)。滑膜内神经末梢分布广泛,在滑膜血管周围分布尤为明显。实验组神经纤维分布较对照组密集。结论 NGF与膝关节OA患者疼痛程度呈正相关,其表达增高可能是患者疼痛加重及膝关节功能障碍的原因之一。
Objective To compare the expression of nerve growth factor(NGF)and distribution of nerve fiber in knee synovium collected from knee osteoarthritis patients and knee trauma patients.Methods NGF expression was detected using semiquantitative immunohistochemistry in knee osteoarthritis and knee trauma patients.Gless staining was adopted to observe the distributional difference of nerve fiber in synovium of the knee.Results NGF mainly expressed in vascular smooth muscle of synovium.NGF expression in experimental group(positive blood vessel 70.97%) was higher than control group(positive blood vessel 37.85%).In experimental group,NGF expression in patients with high pain score was higher than that in patients with low pain score(P0.01);and NGF expression was higher in patients with poor joint function than that in patients with good joint function(P0.01).Never fibers existed widely in synovium of the knees,especially around the blood vessel of synovium.The distribution of never fibers in synovium of experimental group was more intensive than that in control group.Conclusion NGF expression and level of pain is positive correlated in patients with knee osteoarthritis.Enhanced expression of NGF may contribute to progressive pain and joint dysfunction.
