64层螺旋CT在头颈部联合CTA扫描方案中的优化设计 被引量:20

摘要 目的探讨头颈部血管CTA扫描理想的触发阈值以及与之匹配的延迟时间。方法 300例头颈部血管受检者,前瞻性随机分A、B、C 3组,每组100例,分别设计3组不同的阈值及延迟时间,记录3组病例的阈值、达峰时间及延迟时间,剔除血管和颅脑有病变的病例,从每组剩下未见明显异常病变的病例中随机抽取40例,对3组数据得到的图像质量进行评分。结果经过统计学Ridit检验,B组方案扫描明显优于A、C组,所得影像图像伪影少,质量高,能应用于实际工作,减少医师后处理时间。结论造影剂总剂量70ml,注射速度5 ml/s,阈值设定为100 Hu,延迟时间为5 s均能获得理想的图像质量。优化后的扫描方案节约了造影剂的量,提高了图像质量,为临床诊断提供更为准确的依据。 Objective To explore the ideal trigger threshold and matching delay time of head and neck vascular CTA scanning use by 64-slice CT.Methods 300 patients were divided in need of cephalic and cervical vessel examination into A,B,C groups,100 in each group and designed three different threshold values and delay times.The thresholds,the peak times and the delay times of each group were measured and written down and excluded angiopathy and brain-illness cases.40 cases in each group were randomly selected from the normal cases left and the image quality parameters of the three groups were compared by statistics.Results The scanning scheme of group B was much better than that of group A and group C by statistical RIDIT test,whereas the image artifact of group B was less and the image quality was higher,which can be applied to practical work and reduce time taken by doctors in post process.Conclusion The ideal image quality can be obtained if the total amount of contrast medium is 70ml,the injection rate is 5ml/s,the threshold value is set at 100 Hu and the delay time is at 5s.The optimized scanning scheme has saved the amount of contrast medium,improved the image quality and provided a more reliable evidence for the clinical diagnosis.
