摘要
目的探讨多层螺旋CT颈椎扫描重建在推拿手法治疗神经根型颈椎病疗效评价中的价值。方法 30例临床确诊的神经根型颈椎病患者,在推拿手法治疗前后分别进行颈椎多层螺旋CT检查,观察两次检查颈椎的椎间孔变化情况,并结合临床症状及体征进行分析。结果推拿治疗前所观测的300个椎间孔中,发生狭窄的有80个,推拿治疗后狭窄的椎间孔有35个。推拿治疗前后狭窄椎间孔的平均前后径分别为(2.16±0.47)mm和(3.09±0.80)mm,两者差异具有显著统计学意义(P=0.000;t=-11.10)。左侧C2-3、C4-5、C6-7椎间孔的上下径,左侧C5-6椎间孔前后径,右侧C4-5、C5-6椎间孔的前后径,C5-6椎间孔上下径在治疗前后的变化有统计学差异(P<0.001)。结论推拿治疗可以改善椎间孔大小,改善神经根型颈椎病的症状,从而达到治疗目的。多层螺旋CT颈椎扫描重建对神经根型颈椎病的疗效评价具有重要参考价值。
Objectives To analyze the clinical efficacy of tuina therapy in the cervical spondylotic radiculopathy by multi-slice spiral CT(MSCT) imaging and reformation.Methods Image data of 30 patients with cervical spondylotic radiculopathy were collected.All patients were performed twice cervical multi-slice spiral CT study before and after the tuina therapy.The changes of intervertebral foramen in both MSCT studies were observed.These changes were comparatively analyzed with clinical symptoms and signs.Results There was 80/300 narrow intervertebral foramen before treatment,the number was 35/300 after tuina therapy.Before and after massage therapy,the average transverse diameters of those stenosis foramen were(2.16±0.47)mm and(3.09±0.80)mm,there was significant difference between them(P=0.000;t=-11.10).The left C2-3,C4-5,C6-7 foraminal longitudinal,left C5-6 intervertebral foramen,right C4-5,C5-6 intervertebral foramen,the upper and lower intervertebral foramen diameter of C5-6 changes before and after tuina therapy were significantly different(P0.001).Conclusions Tuina therapy is effective for cervical spondylotic radiculopathy by improving the cervical curvature,foraminal size and the relationship of facet joint position.Multi-slice CT imaging and reformation have an important referential value in the evaluation of clinical efficacy of tuina therapy in cervical spondylotic radiculopathy.
