IMA与AMI冠状动脉病变程度的临床关系 被引量:1

摘要 目的探讨缺血修饰白蛋白(IMA)对急性心肌梗死(AMI)冠状动脉病变程度及临床状态的诊断价值。方法胸痛发作3h内到达医院的AMI患者,采血测定IMA,描记12导联心电图(12-ECG),入选患者均规范诊断、危险分层、标准治疗,行经皮冠状动脉造影,应用Gensini计分法计算其积分以反映冠状动脉病变程度,所有入选患者根据临床killip分级及Gensini积分分组,比较IMA组间差异。结果不同Gensini积分组间IMA值比较差异无统计学意义;killip1级组与killip>1级组间IMA值比较差异有统计学意义;经受试者工作曲线(ROC)分析获得最适界值(cutoff)点(IMA=83.22),IMA预测急性心肌梗死发生急性心力衰竭的敏感度78.9%,特异度72.4%。结论 IMA可早期诊断急性心肌梗死,并预测心梗后急性心力衰竭的发生,但IMA水平不能反映冠状动脉病变严重程度。
