人眼玻璃体基底-视网膜界面超微结构与锯齿缘解离

摘要 目的通过对人眼玻璃体基底-视网膜界面超微结构的研究,认识基底部玻璃体与视网膜的连接方式及特点,探讨锯齿缘解离的发病机制。方法对4例锯齿缘解离患者术中所获标本及10例眼库眼球标本行透射电子显微镜观察,研究玻璃体基底-视网膜界面的超微结构及连接特点。结果锯齿缘解离患者标本及眼库眼球标本均显示玻璃体基底与其下的神经视网膜及视网膜色素上皮紧密粘连。电镜下显示:与其他部位相比,玻璃体纤维明显粗大密集,呈束状、发辫状、编织状排列,视网膜内界膜缺损或消失,玻璃体纤维插入其下,或穿入细胞层的"隐窝",与视网膜神经胶质细胞(Mller细胞)甚至视网膜色素上皮细胞形成错综复杂的紧密连接。同时,在玻璃体基底-视网膜界面还存在有"类半桥粒"样结构的紧密连接方式。结论玻璃体基底-视网膜界面处复杂的紧密连接方式是导致锯齿缘解离发生的解剖学基础。 Objective Through studying the ultrastructures of the vitreous basement-retinal interface in human eyes,to realize the connection mode and characteristic and to discuss the pathogenesis of ora serrata dialysis.Methods Four samples of ora serrata dialysis were obtained during the operations,and the other 10 samples were obtained from fresh cadaver eyes.The ultrastructures of the vitreous basement-retinal interface and its connection characteristic in the samples were studied with transmission electron microscope.Results The collagen fibrils in the basal vitreous were at a higher concentration and arranged in bundles and braiding,the internal limiting membrane defected or disappeared,a complex network of fibrils merged on the cellular side of internal limiting membrane or passed into "crypts" within the cellular layers,some of them even attached to the surface of retinal pigment epithelium.The structures similar to the hemidesmosome were also found at the vitreous basement-retinal interface.Conclusion The tight adhesions of vitreous basement-retinal interface in human eyes play an important role in the occurrence of ora serrata dialysis.
