LASIK术中不同制瓣方式对比敏感度的变化 被引量:7

Changes of contrast sensitivity of corneal flap creation with different ways in LASIK

摘要 目的对比分析准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)术中飞秒激光制瓣与传统微型角膜板层刀制瓣后对比敏感度变化。方法 Ziemer LDV飞秒激光制作角膜瓣组20例40眼患者作为飞秒组,传统微型角膜板层刀制作角膜瓣20例40眼患者作为板层刀组。所有病例使用同一台准分子激光机。比较术后2组患者的视力及对比敏感度。结果 LASIK术后1周,白天和夜晚状态下2组各空间频率的对比敏感度均较术前有明显下降,差异均有统计学意义(均为P<0.05),至术后3个月时,2组各空间频率对比敏感度恢复至术前水平。飞秒组夜晚带眩光状态各空间频率术后1周、术后1个月、术后3个月均高于术前水平,差异均有统计学意义(均为P<0.05)。板层刀组夜晚带眩光状态各空间频率术后1周下降,至术后1个月、3个月高于术前水平,差异均有统计学意义(均为P<0.05)。结论 LASIK术后对比敏感度会出现短暂下降,但是飞秒激光制瓣相对于板层刀制瓣术后对比敏感度恢复较快。 Objective To investigate the changes of contrast sensitivity of corneal flap creation with a femtosecond laser and a mechanical microkeratome in LASIK.Methods The corneal flap was created by Ziemer femtosecond laser in 20 patients(40 eyes)as the femtosecond laser group,corneal flap with a mechanical microkeratome in another 20 patients(40 eyes)as the hansatome group.All eyes received wavefront-guided LASIK treatments performed with Technolas 217z100 excimer laser.The changes of visual acuity and contrast sensitivity after operation were research in two groups.Results At postoperative 1 week,the photopic and mesopic contrast sensitivity function in two groups decreased for all spatial frequencies,there were statistical differences compared with that before operation(all P〈0.05),till postoperative 3 months,the contrast sensitivity function in two groups restored to preoperative level.The mesopic contrast sensitivity with glare condition in all spatial frequencies at postoperative 1 week,1 month and 3 months in the femtosecond laser group were higher than that before operation,there were statistical differences(all P〈0.05).The mesopic contrast sensitivity with glare condition in all spatial frequencies at postoperative 1 week in the hansatome group decreased,but till the postoperative 1 month,3 months,which was higher than that before operation,there were statistical differences(all P〈0.05).Conclusion The contrast sensitivity will have a transient decrease after LASIK,but femtosecond laser results in a faster recovery than mechanical microkeratome.
