嗜麦芽窄食单胞菌家兔感染动物模型体内药物敏感性监测

Establishment on rabbit model of Stenotrophomonas maltophilia infection and antimicrobial sensitivity monitoring in vivo

摘要 目的:建立嗜麦芽窄食单胞菌感染的家兔细菌感染模型,观察抗菌药物在家兔体内的药物敏感性,比较何种体外药物敏感试验更能准确反应临床疗效。方法:在60只家兔腹腔中分别接种0.5 m l浓度为0.5麦氏的嗜麦芽窄食单胞菌菌液,从接种菌之时起,将家兔分成3组(A、B、C组),每组20只,每24 h测量家兔体温,共10次,并抽血测量计数白细胞、内毒素;分别在48 h、72 h、96 h、120 h、144 h注射药物:A组注射哌拉西林/他唑巴坦(TZP);B组注射替卡西林/棒酸(TIM);C组注射头孢吡肟(PEF)。比较M IC法、纸片琼脂扩散法(K-B法)及仪器法检测的药物敏感性结果与体内药物治疗的疗效关系。结果:成功建立了家兔的细菌感染模型,TZP治疗组(A组),M IC法与体内治疗符合率83.3%;VITEK法与体内治疗符合率77.8%;K-B法与体内治疗符合率55.6%。TIM治疗组(B组),M IC法与体内治疗符合率77.8%;VITEK法与体内治疗符合率77.8%;K-B法与体内治疗符合率44.4%。PEF治疗组(C组),M IC法与体内治疗符合率88.9%;VITEK法与体内治疗符合率77.8%;K-B法与体内治疗符合率66.7%。结论:本研究中家兔体温、内毒素含量、白细胞计数是最有效价判断家兔感染的指标。在已用的检测法中,对3种药物的体外敏感性检测结果与家兔体内药物敏感性符合率最高的方法均为M IC法,建议使用M IC法。 Objective:To build the rabbits infection model of the Stenotrophomonas maltophilia,then with Piperacillin-Tazobactam,Ticarcillin-clavulanate,Cefepime cure to monit drug sensitivity in vivo,to pick out the best drug sensitivity test in vitro to show curative effect.Methods: Rabbits were divided into three treatment groups(A,B and C),19 injected with 0.5 ml Stenotrophomonas maltophilia in every group as the density is 108 cfu·ml-1,one injected with 0.5 ml NS in every group as the negative control.After inoculation,the rabbits'anus temperature and leucocyte and Endotoxin(Lap) were determined every 24 hours,and at 48 h、72 h、96 h、12 h、144 h,1 ml of 2 g/ml Piperacillin-Tazobactam was injected in A group for the rabbits injected with 0.5 ml Stenotrophomonas maltophilia.In the same way,for B group with 1 ml of 0.6 g/ml Ticarcillin-clavulanate,for C group with 1mL of 0.1 g/ml Cefepime,then use paper agar diffusion method(K-B mdthod) and MIC method and VITEK method for the 57 strains in vitro drug sentivity test.Results: Rabbit models of Stenotrophomonas maltophilia infection were successfully established,and test results consistent with animal modles of course of treatment rate in the group A were 83.3% in MIC method,55.6% in KB method and 77.8% in VITEK method.In the group B were 44.4% in K-B method,77.8% both in MIC method and in VITEK method.In the group C were 66.7% in K-B method,88.9% in MIC method and 77.8% in VITEK method.Conclusion: Body temperature and Endotoxin(Lap) to be the most effective price indicators of infection was determined,in the detection method has been used,MIC method from the animal infection and drug sensitivity in animals is most closely monitored,and there is big difference between rabbits in vitro antimicrobial susceptibility and antimicrobial sensitivity.
