摘要
目的：通过对职业消防员十二指肠溃疡（DU）发病情况的问卷调查及相关客观指标的检测,了解DU在职业消防员这类特殊群体范围内发病率,从中医辨证分型探讨与之密切相关的发病因素.方法：按照消化性溃疡临床诊断标准设计DU症状自评量表[1],采用整群完全随机抽样方法,对消防某部队院校来自全国16个省份的400名官兵（职业消防员的骨干力量）进行DU流行病学调查,按照SPSS13.0统计软件建立数据库,对相关病因开展统计分析.结果：职业消防员DU发病率约为23.25%.统计量显示情志、饮食、劳倦因素,工作及生活环境影响,是否患有＂神经官能症＂、＂胃肠炎＂、＂贫血＂、＂疼痛性疾病＂等慢性病,对证型差异有显著性意义（P〈0.05）.而性别、籍贯、民族、学历、年龄、服役时间等个人情况,吸烟、饮酒、失眠等一般因素,相关实验室指标结果,对证型差异无显著性意义（P〉0.05）,DU患者的Hp感染率为84.95%.结论：通过分析,职业消防员患DU的病因以情志失调为主,饮食伤胃和劳倦过度次之,与Hp感染因素一起,促发消防战士中较高的DU发生率.
Objective of the Professional Fire duodenal ulcer （DU） survey of the incidence of objective indicators and related testing, career firefighters about DU in the context of these special groups, type of syndrome differentiation from the closely related risk factors. Method：in accordance with the design of clinical diagnosis of peptic ulcer DU SCLE13 , using completely random cluster sampling method, a military academy on fire from all 16 provinces of the 400 officers and men （the backbone of professional firemen ） for DU epidemiological investigation, in accordance with the SPSS13. 0 statistical software, databases, conduct statistical analysis on the related causes. The results：The incidence of professional firefighters is approxi- mately 23.25~/0 DU. Statistics show emotions, diet, Laojuan factors affect the working and living environment, whether suffering from ＂neurosis＂, ＂gastroenteritis＂, ＂anemia＂, and ＂pain disease＂ and other chronic diseases, check against type difference was significant significance （P^0.05）. However, gender, ethnicity, nationality, education, age, length of service and other personal circumstances, smoking, drinking, insomnia and other general factors, the results of relevant laboratory parameters, type check against no significant difference （P〉 0.05）. DU patients with Hp infection rate was 84.95%. Conclusion The analysis of the etiology of DU career firefighters suffering from emotional disorders in the main, over--eating fol- lowed by Shang Wei and Laojuan, together with Hp infection factors, trigger fire fighters in the higher incidence of DU.
