切开复位Herbert钉内固定术治疗MasonⅢ型桡骨小头骨折 被引量:2

摘要 目的总结Herhert钉治疗MasonⅢ型桡骨小头骨折的临床疗效。方法 22例MasonⅢ型桡骨小头骨折患者,采用切开复位Herbert内固定,对其中18例随访资料完整的病例进行回顾性分析。结果随访18例,平均21(12~36)个月。所有骨折愈合良好,平均愈合时间6(4~10)个月,未发生肘关节不稳或功能减弱。平均Mayo肘关节性能指标评定分数为88.6±11.4,临床疗效满意:优12例,良6例。肘关节平均屈伸范围7°~135°,旋前平均80°(73°~87°)和旋后平均72°(66°~78°)。结论 Herbert钉对于选择性的治疗MasonⅢ型桡骨小头骨折能够获得满意的临床效果,固定牢固可靠,可早期功能锻炼。 Objective To evaluate the clinical effect of Herbert screws internal fixation of the Mason type Ⅲ radial head fractures.Method 18 radial head fracture patients with Mason type Ⅲ were treated by open reduction and Herbert screw internal fixation.Results 18 cases were followed up for 21 months averagely(ranged from 12 to 36 months).All fractures healed well in 4 to 10 months after operation.No elbow instability or weakness occurred.The mean Mayo Elbow Performance Index score was 88.6±11.4,the results showed excellence in 12 and good in 6.The flexion range of the elbow was 7° to 135°,with 80° pronation and 72° supination averagely.Conclusion Internal fixation of Herbert screws can achieve satisfactory effect for Mason type Ⅲ radial head fracture with advantages as rigid internal fixation and early function exercises,but the indication should be strictly selected.
