摘要
目的:探讨乳腺癌超声征象与分子亚型的关系。方法:回顾分析自2009年3月至12月经病理检查证实的128例乳腺癌患者,均有有雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER2)免疫组织化学检测等资料及术前行超声检查的超声图像,以免疫组织化学技术为基础,将乳腺癌分为3个分子亚型,即三阴性型(ER、PR、HER2均阴性)、Luminal型(ER阳性或PR阳性,HER2阴性或阳性)和HER2过表达型(ER阴性、PR阴性、HER2阳性)。结果:3个亚型的乳腺癌其超声征象在肿块的形状、生长方位、边界、边缘、后方回声特征、是否存在微钙化方面差异均有统计学意义(P<0.05);三阴性型乳腺癌多边界清晰(85.3%),边缘规则(47.1%),后方不伴回声衰减(23.5%)及较少出现微钙化(32.4%);HER2过表达型相对于Luminal型及三阴性型更易出现微钙化(56.5%),差异有统计学意义(P<0.05);Luminal型相对其他亚型更易出现高回声晕(18.3%),差异亦有统计学意义(P<0.05)。结论:乳腺癌超声表现与其分子亚型间存在相关性。
Objective To study the correlation between ultrasonographic features and molecular subtypes of breast cancer.Methods Ultrasounographic findings of 128 patients with pathologically confirmed breast cancer from March to December 2009 were retrospectively analyzed.Patients were categorized into 3 molecular subtypes : triple-negative group [ER(-),PR(-) and HER2(-),n=34],Luminal group [ER(+) or PR(+),with HER2(-) or HER2(+),n=71],HER2 group [ER(-),PR(-) and HER2(+),n=23].Results Different molecular subtypes differed in tumor shape,tumor orientation,margin,boundary,posterior echo and microcalcification on ultrasonography(P0.05).Triple-negative breast cancers were more likely to have clear boundary(85.3%),circumscribed margin(47.1%) and were less associated with posterior acoustic shadowing(23.5%) and microcalcification(32.4%) on ultrasonography.Microcalcification was more frequently seen in HER2 group(56.5%)when compared with triple negative group and Luminal group.Luminal group was most likely to have echogenic halo(18.3%)(P0.05).Conclusions Correlation exists between ultrasonographic features and molecular subtypes of breast cancer.
