摘要
目的 探讨红霉素对术后腹腔粘连及粘连性肠梗阻的预防作用。方法 采用两种腹腔粘连动物模型,将36 只大鼠随机分为红霉素组(8 mg/kg ,首次舌静脉注射,以后1 次/8 h ,背部皮下注射,连用3 d) 和对照组。术后第9 d 处死动物,入腹观察腹腔粘连情况。另外,将124 例腹部手术患者随机分为红霉素组(1 g/d ,连用3 d ,静脉滴注) 和对照组,分别记录肠鸣音恢复时间、排气时间和排便时间,随访18 个月,观察粘连性肠梗阻的发生率。结果 在两种模型中,红霉素组的粘连得分及粘连百分比均显著低于对照组( P < 0 .01) 。红霉素组患者的肠鸣音恢复时间、排气时间、排便时间及粘连性肠梗阻的发生率均明显低于对照组( P < 0 .01 , P < 0 .025) 。结论 红霉素可预防腹腔粘连及粘连性肠梗阻的发生,利于肠功能的恢复。
Objective To investigate the preventive effects of erythromycin (Ery) on postoperative peritoneal adhesions and adhesive intestinal obstruction. Methods Abstract Objective To investigate the preventive effects of erythromycin (Ery) on postoperative peritoneal adhesions and adhesive intestinal obstruction. Methods The adhesions were induced at laparotomy by two different operative models. Thirty six rats were randomly divided into Ery group (Ery, 8?mg/kg, sc, q8?h) and control group. All animals were reexplored at the 9th day postoperatively for evaluation of adhesion formation. One hundred and twenty four patients with laparotomy were randomly to divided into Ery group (Ery, 1?g/d, iv) and control group. The recovery time of bowel sound, the exsufflating time and the defecating time were recorded. All cases were followed up for 18 months and the occurrance rate of adhesive intestinal obstruction was calculated. Results In the two models, the adhesive scores and adhesive percentage in Ery group were significant lower than those in control group (P<0.01). It was found that the recovery time of bowel sound, exsufflating time, defecating time and the occurrance rate of adhesive intestinal obstruction were markedly decreased in Ery group comparing with the control group (P<0.01, P<0.025). Conclusions These findings suggest that Ery may prevent the postoperative peritoneal adhesion formation and the occurrance of adhesive intestinal obstruction, and it is helpful to the recovery of intestinal function with less side effects.
