摘要
目的通过对乳腺癌新辅助化疗前后超声各指标的变化进行观察分析,探讨彩超在乳腺癌新辅助化疗疗效评价中的应用价值。方法 106例乳腺癌患者110处乳腺癌病灶,均于新辅助化疗前后应用彩超对比分析肿瘤原发灶大小和形态,原发灶内血流分级及血流动力学变化及腋下淋巴结形态和血流变化。结果超声显示乳腺癌新辅助化疗后原发灶肿瘤体积显著缩小,形态变的不规则,边界多清晰可见,周边声带可消失,后方回声倾向正常。同时原发灶内血流减少甚至消失,血流动力学指数降低,淋巴结体积缩小,血流减少。结论新辅助化疗后,乳腺癌原发灶及淋巴结的彩超多项指标发生了显著的变化,因此彩超是评价乳腺癌新辅助化疗疗效的有效,无创,安全的检查方法 。
Objective To investigate the value of color doppler ultrasound changes of breast cancer in evaluation of therapeutic effect of neoadjuvant chemotherapy.Methods 106 patients with 110 breast cancers before and after neoadjuvant therapy were examined with color doppler ultrasound.The tumor position,size,configuration,the blood flow and hemodynamics index of the tumor,the axillary lymph nodes including the size,shape and internal blood were examined.Results After neoadjuvant chemotherapy,there was a significant decrease of tumor size,volume,morphology irregularity,existence of surrounding halo,distinct boundary,increasing of internal echoes,rear echo.At the same time,the blood flow and hemodynamics index of the tumor,the size,shape and internal blood of the axillary lymph nodes were also reduced.Conclusion The features of color doppler ultrasound changed significantly after neoadjuvant chemotherapy.Color doppler ultrasound provides a safe,effective and noninvasive method for observing the curative effect of neoadjuvant chemotherapy.
