缺氧缺血性脑病患儿IL-6和IL-18动态检测的意义 被引量:1

Significance of dynamic detection of interleukin-6 and interleukin-18 in children with hypoxic ischemic encephalopathy

摘要 目的:探讨缺氧缺血性脑病患儿IL-6和IL-18动态检测的临床意义。方法:选择诸暨市妇幼保健院新生儿科收治的缺氧缺血性脑病患儿48例作为实验组,同时选择54例同期在该院产科正常分娩的足月新生儿作为对照组,分别于出生后24 h、第3天和第7天取实验组和对照组新生儿的外周静脉血,采用ELISA法进行血清中细胞因子IL-6和IL-18水平的测定,并对新生儿出生24 h内的静脉血进行大脑中动脉的血流动力学参数的检测。对两组新生儿不同时期的IL-6和IL-18水平以及血流动力学参数进行比较分析。结果:与健康正常组新生儿相比,实验组患儿IL-6和IL-18水平在24 h、3天和7天时均明显降低,且实验组患儿脑血流速度参数收缩期峰值流速(PSFV)、舒张期末血流速度(EDFV)、时间平均流速(TMFV)以及搏动指数(PI)均明显减慢,差异具有统计学意义(P<0.05)。结论:缺氧缺血性脑病患儿的IL-6和IL-18具有一定的变异性,因此对新生儿进行IL-6和IL-18水平的检测对缺氧缺血性脑病的诊断及治疗具有十分重要的意义。 Objective:To explore the clinical significances of dynamic detections of interleukin-6(IL-6) and interleukin-18(IL-18) in children with hypoxic ischemic encephalopathy(HIE).Methods:48 children with HIE hospitalizing in department of neonatology in the hospital were selected as experimental group,54 full term neonates of normal delivery during the same period were selected as control group,the venous blood samples of experimental group and control group were abstracted at 24 hours,3 days and 7 days after birth,ELISA method was used to detect the serum levels of IL-6 and IL-18,the venous blood samples within 24 hours after birth were abstracted to detect the hemodynamic parameters in middle cerebral artery.The levels of IL-6 and IL-18 during different periods and hemodynamic parameters in the two groups were compared and analyzed.Results:Compared with control group,the levels of IL-6 and IL-18 at 24 hours,3 days and 7 days after birth decreased significantly,and the hemodynamic parameters including peak systolic flow velocity(PSFV),end diastolic flow velocity(EDFV),time average flow velocity(TMFV) and pulsatility index(PI) all decreased significantly(P0.05).Conclusion:IL-6 and IL-18 levels in the children with HIE have variability to a certain extent,thus,detections of IL-6 and IL-18 have great significance for the diagnosis and treatment of neonates with HIE.
