复杂胫骨平台骨折三柱分型及治疗分析 被引量:6

摘要 目的:探讨复杂胫骨平台骨折三柱分型方法以及对其治疗效果的分析。方法:根据基于CT重建为基础的三柱分型方法,将35例复杂胫骨平台骨折分为双柱骨折23例,三柱骨折12例。所有病例均采用手术治疗,采用合适手术入路及钢板内固定复位骨折块,均取自体髂骨进行植骨。结果:35例患者均获得随访,平均随访21.8月。术后即刻X线片示33例解剖复位,2例复位不完全。全部骨折均临床愈合,无骨不连,内固定松动断裂等并发症发生。本组资料显示优23例,良9例,可2例,差1例,优良率91.4%。结论:利用三柱分型方法对复杂胫骨平台骨折可以起到良好的手术指导作用,切开复位内固定手术治疗复杂胫骨平台骨折可以取得良好的治疗效果。 Objective: To discuss the three column classification of complicate tibial plateau fractures and their treatment.Methods: Thirty-five patients with complicate tibial plateau fractures were divided into two column fractures(23 cases) and three column fractures(12 cases) according to a CT-based three dimensional rebuilding system—"Three-Column Classification".All patients were treated with surgery via individual approach and fractures were repaired with internal fixation with self planted bones.Results: An average of 21.8 months of follow up was conducted on all patients,and X-ray showed that anatomical restoration of bones in 33 cases and dislocation in 2 cases.All cases were clinically recovered,no complications like loose fixation was observed.Excellent effects was observed in 23 cases,good in 9,ok in 2,poor in 1 case showing a good land excellent rate of 91.4%.Conclutions: The three column classification method is good for the treatment of complicate tibial plateau fractures.
