摘要
目的 探讨医源性胆管损伤多次手术的原因及防治对策。方法 对我科过去１０ 年来收治的医源性胆管损伤３次以上手术者２１ 例进行回顾性分析。结果 医源性胆管损伤多次手术原因与手术技术不当，伤后未能早期发现，二次手术时机选择欠佳，支撑引流时间太短等有关。结论 胆管损伤后即时诊断及早期合理处置是避免医源性胆管损伤后多次手术的关键；胆道多次手术者胆道重建宜选胆肠Ｒｏｕｘ－ Ｙ吻合术，疗效满意。
Objective To investigate the causes and treatment of the repeated operations on the iatrogenic bile duct injury.Methods Twenty-one patient with three times operations and more on the iatrogenic bile duct injury ocuurred in the past 10 years in our department were studied retrospectively.Results The causes of the repeated operation on the iatrogenic bile duct injury were associated with incorrect operative procedure,no discovery the injury in the primary operation,inproper reoperative time and short stent with T tube in the repaired bile duct.Conclusion The authors suggest that early diagnosis and appropriate management for the iatrogenic bile duct injury can prevention the repeated bile duct operation and improve the clinical results,However,for patient whose treatment was delayed for several days or reconstruction surgery failed,the Roux-Y procedure is more reliable.
