超声引导下细管经皮穿刺肾造瘘术的临床应用

摘要 目的:观察超声引导下14G细管经皮穿刺肾造瘘术(percutaneous nephrostomy,PCN)的可靠性及安全性,对比其与传统方法的优劣。方法:对32例恶性肿瘤、结石等原因导致急性梗阻性肾功能不全的患者,在超声引导下进行14G细管径引流管PCN,与20例F16硅胶引流管PCN患者对比研究,观察肾功能恢复情况及并发症。结果:32例14G细管PCN患者首次置管造瘘成功,所有病例术后3 d内肾功能得到明显改善,未出现严重并发症。20例F16硅胶引流管PCN患者血尿时间延长,3例出现肾周血肿。结论:超声引导下细管经皮穿刺肾造瘘术可达到理想的引流效果,与粗管引流无明显差异,并发症发生率低,为解决肾后性肾功能不全提供了一种微创、有效的方法。 Objective To investigate the achievement ratio and safety of the use of tubule in ultrasound-guided percutaneous nephrostomy in patients with postrenal renal inadequacy.Method Total 32 patients with postrenal renal inadequacy underwent percutaneous nephrostomy were guided by ultrasonography with 14G tubule,and 20 patients with F16 silica gel tube.Results Percutaneous nephrostomy were successful in 32 patients at first attempt with 14G tubule.The improvement of renal function was marked,and no severe complications occurred.Conclusion Ultrasound-guided percutaneous nephrostomy with tubule is a safer procedure for patients with obstructive uropathy compared with traditional measure.
