相干光断层扫描仪对超声乳化术后透明角膜切口构型的分析 被引量:1

Evaluation of clear corneal incision after cataract surgery by using optical coherence tomography

摘要 目的通过相干光断层扫描仪(OCT)对超声乳化术透明角膜切口进行组织结构评估。方法 80例(81眼)进行超声乳化手术的患者均采用颞侧2.6mm透明角膜切口,术后24h,采用傅立叶OCT对角膜切口闭合状况进行检查,测量眼压并做渗漏试验。结果所有角膜切口均闭合,未见渗漏;术后平均眼内压为(14.97±4.91)mmHg(1mmHg=0.133kPa)。透明角膜切口平均长度为(1.95±0.23)mm,切口平均角度为(50.90±7.10)°。所有切口均存在内皮面未闭合;38.27%的切口上皮面未闭合,69.14%的切口内弹力层剥脱,19.75%的切口基质面未闭合。伴有与未伴有透明角膜切口构型不良事件组的切口长度、切口角度与眼内压之间差异无统计学意义(P>0.05)。结论透明角膜切口在超声乳化术后24h存在不同程度构型问题,OCT能较好地观察术后透明角膜的切口构型。 Objective To access the configuration of the most favorable sutureless clear corneal incision (CCI) after cataract surgery by using Fourier-domain optical coherence tomography (OCT). Methods Eighty-one eyes from Eighty patients treated with cataract surgery with 2.6 mm temporal CCI were evaluated for the profile of the temporal CCI 24 h after the surgery. All patients were examined for the intraocular pressure (IOP) and tested with the leaking test. The architecture was described according to the angle and the length of incision, apposition of epithelial or endothelial margins, Descemet' s membrane detachment and loss of apposition of the incision. Results No leakage was detected. The mean lOP was (14.97±4.91) mm Hg (1 mm Hg=0. 133 kPa). The mean incision angle was (50.90±7.10)°; the mean incision length was (1.95± 0.23 ) mm. hnperfect appositions of the endothelial margin were seen in all cases; incomplete sealing of the epithelium was seen in 38.27% of the cases. Localized Descemet' s membrane detachment in the vicinity of the wound was found in 69.14% of the cases. Loss of apposition of the incision was found in 19.75% of the cases. No statistical correlation was found between these anatomic imperfections of CCI in the incision' s angle or length or the IOP. Conclusions OCT revealed the imperfect architectural integrity of the CCI at 24 h after cataract surgery.
