摘要
目的探讨血清同型半胱氨酸(HCY)、踝臂指数(ABI)和肱踝脉搏波传导速度(baPWV)与冠心病的关系。方法选择168例疑似冠心病患者,根据冠状动脉造影结果分为冠心病组(116例)和非冠心病组(52例),冠心病组又分为单支病变组45例、双支病变组43例、多支病变组28例。采用荧光偏振免疫分析法(FPIA)测定HCY,VP1000动脉硬化检测仪检测ABI、baPWV。结果冠心病组的HCY、baPWV高于非冠心病组,差异均有统计学意义(P<0.05);ABI低于非冠心病组(P<0.05);多支病变组的baHCY水平高于单支及双支病变组(P<0.05);双支、多支病变组的baPWV高于单支病变组,差异均有统计学意义(P<0.05);多支病变组的ABI低于单支、双支病变组(P<0.05)。结论 HCY、ABI、baPWV与冠心病及其病变程度有关,三者联合可用来评估冠心病及其危险程度,从而更有效地防治冠心病的发生发展。
Objective To explore the associations of coronary heart disease(CHD) with serum homocysteine(HCY) levels,ankle-brachial index(ABI),and Brachial-ankle PWV(baPWV).Methods A total of 168 patients with suspected CHD were divided into CHD group(116 cases) and non-CHD group(52 cases) according to coronary arteriography results,with the CHD group further divided into single-branch-affected(SBA) group(n=25),two-branch-affected(TBA) group(n=31) and multi-branch-affected(MBA) group(n=49).Serum HCY levels were measured by the fluorescence polarization immunoassay(FPIA) and baPWV and ABI were assessed non-invasively using a VP 1000 automated PWV/ABI analyzer(PWV/ABI,Colin Co.Ltd.,Komaki,Japan).Results The CHD group showed higher levels of HCY and baPWV and lower ABI levels than the non-CHD group(P0.05);HCY levels tended to increase with the number of vessels affected,with higher levels in the MBA group than those in the SBA and the TBA group(P0.05);the TBA group and MBA group showed higher baPWV than the SBA group(P0.05);the MBA group showed lower ABI than the SBA and TBA groups(P0.05).Conclusion Levels of HCY,ABI and baPWV,associated with the existence and degree.of CHD,could be applied to,combined,assessment of the severity and risk of CHD in order to prevent the pathogenesis and development of the disease more effectively.
