广西百色市3个民族人群RhD阴性和弱表达D表型分布的比较研究 被引量:5

摘要 目的对广西百色市壮、汉、苗族人群的Rh血型分布进行调查并与弱表达D表型的分布进行比较研究。方法 ABO血型采用正定玻片法,Rh血型采用Rh抗D单克隆抗体测定,另外对RhD阴性个体应用聚合酶链反应(PCR-SSP)技术检测RhD基因外显子和RhCE基因。结果居住在广西百色市的壮、汉、苗族人群RhD阴性率分别为3.86%、0.93%、8.79%,d基因频率分别为0.187、0.079、0.215。壮族人群RhD阴性血清学表型分布极不平衡,以cde多见,占56.1%,Ccde次之,占29.1%,其他表现型频率较低;34例弱D标本血清学表型主要为cDEe(47.1%)和CcDe(38.2%),其他表型所占频率较少。结论广西百色市壮、苗族等少数民族人群的Rh血型比例较高,可能与本地区Rh溶血症发生率相对较高相关,因此常规检查Rh血型对避免Rh血型不合引起的溶血性输血反应、新生儿溶血具有重要意义。 Objective To compare and analyse the distribution of negative RhD and weak expression D of three nationalities in Guangxi Baise city.Methods ABO blood type group was tested by slide method,while RH blood type group was tested by anti-D-dimer antibody,and RhD gene exon,RhCE gene of RhD negative persons were tested by PCR-SSP.Results The positive rates of RhD gene of Zhuang,Han,Miao nationalities in Baise Guangxi were 3.86%,0.93%,8.79%.The d genetic frequencies were 0.187,0.079,0.215 respectively.The serology phenotype distribution of RhD negative Zhuang nationality was extremely imbalance.The phenotype frequency of cde was the highest,accounted for 56.1%,followed by the Ccde,accounted for 29.1%,others phenotype frequencies were low;The serology phenotype of 34 cases of weak D samples were mainly cDEe(47.1%)and CcDe(38.2%),others phenotype frequencies were low.Conclusion The rate of Rh blood type is high in ethnic minority such as Zhuang,Miao nationality in Baise Guangxi.This may be associated with the high incidence of Rh haemolysis in this area.To prevent the transfuse reactions and HDN caused by Rh-incompatibility,we should check the Rh blood type by routine examinations.
