Surgical techniques of posterior fossa craniotomy: report of 32 cases

摘要 目的总结后颅窝肿瘤骨瓣成形术的手术操作技巧,提高临床治疗效果。方法回顾性分析32例后颅窝肿瘤的临床资料,本组采用枕下正中/旁正中入路10例,枕下外侧乙状窦后入路22例,所有病例均采用骨瓣开颅,术后骨瓣复位固定。结果肿瘤全切除31例,次全切除1例;术中3例乙状窦损伤经修补后恢复良好;术后切口部位轻度疼痛2例,皮下积液1例,无脑脊液漏及假性脑膜膨出。结论后颅窝骨瓣开颅安全、有效,能明显减少后颅窝骨缺损相关的并发症。 Objective To summarize surgical techniques of posterior fossa craniotomy for posterior fossa tumors and to improve clinical outcomes. Methods Clinical data of 32 cases of posterior fossa tumors were retrospectively analyzed. Ten cases received tumor resection via suboccipital midline or para - midline approach, while the other 22 via retrosigmoid approach. Craniotomy was applied in all cases and bone flaps were secured in situ. Results Total tumor removal was achieved in 31 cases, with subtotal removal in 1 case. Three cases with retrosigmoid sinuses injured and repaired during the surgical procedure recovered well. Postoperative local pain related to the healed wound and subcutaneous hydrops occurred in 2 and 1 cases, respectively. No cerebrospinal fluid rhinorrhea or pseudomeningocele was observed. Conclusion Posterior fossa craniotomy is a safe and effective technique, with less posterior fossa bone defect - induced complication.
