摘要
目的探讨肾透明细胞癌的CT征象与病理表现之间的关系,并对两者进行对照分析.方法对我院2006-09—2010-09月44例经手术后病理证实的肾透明细胞癌病例术前CT征象及术后病理结果,进行回顾对照分析.结果 CT平扫8例病灶为高密度,17例为低密度,19例为混杂密度.CT强化皮质期43例肿瘤实体强化幅度超过正常肾实质,42例肿瘤强化峰值≥88 HU,其中39例强化峰值≥100 HU,排泄期迅速下降,低于正常肾实质;1例轻度强化,38例肿瘤呈不均匀强化,26例可见假包膜.病理上42例可见出血、坏死、囊变,细胞呈实质性排列,2例呈现囊性排列,39例中出现小泡样结构,1例出现了不规则的钙化;37例可见假包膜.结论肾透明细胞癌的CT表现与病理类型及肿瘤结构有关.肾透明细胞癌CT平扫表现特异性不强,动态增强对肾透明细胞癌的诊断有很大的意义,组织学类型确诊仍需依靠病理.
Objective To study and analyze the relationship between CT findings of clear cell renal cell carcinoma(CCRCC) and results of surgical pathology.Methods 44 cases with clear cell renal cell carcinoma(CCRCC) who were operated in our hospital from Sept.2006 to Sept.2010 were investigated by pathology after operation.CT findings before operation and pathological results after operation were compared.Results Focuses of 8 cases were high density by conventional CT scan,while that of 17 cases were low density and 19 cases were mixed density.Enhancement scan of 43 solid tumors was beyond Kidney in normal.For 42 cases,the peak attenuation was more than or equal to 88HU,and peak attenuation for 39 of them was more than or equal to 100HU,which decreased rapidly in the excretory phase and below the level of Kidney in normal.One case was slight enhancement.38 cases were asymmetrical enhancement.Pseudocapsule was seen on 26 cases,and the blood,died and cystic degeneration were seen on 42 cases.2 cases were cystic permutation.Froth appeared on 39 cases,while 1 case appeared calcification in uncommon rules,and 37 cases were seen pseudocapsule.Conclusion CT appearance of CCRCC has the relationship with the pathologic types and the structure of tumor.Plain CT scan for CCRCC has not enough specificity.Kidney dynamic enhanced scan has more significance for CCRCC.The histological types depend on Pathological examination.
