高频振荡器在大容量全肺灌洗治疗尘肺中的应用研究 被引量:3

摘要 [目的]探讨高频振荡器在大容量全肺灌洗治疗尘肺中的疗效和安全性。[方法]选择单侧分期大容量全肺灌洗治疗尘肺病人125例,按住院顺序随机分为两组。两组病人均在静脉吸入复合全身麻醉下经双腔气管插管行单侧大容量全肺灌洗,对照组术中术者和巡回护士用手反复叩击灌洗侧胸廓而试验组术中使用高频振荡器对灌洗侧胸廓进行辅助振荡。以病人肺功能及临床症状改善评价疗效。[结果]两组病人灌洗前后肺功能及临床症状改变均有统计学意义(P<0.05);试验组灌洗后残余量、肺功能及临床症状改善与对照组比较有统计学意义(P<0.05或P<0.01);住院时间试验组为16.94 d±1.57d,对照组为18.31 d±2.02 d,两组比较差异有统计学意义(P<0.01)。[结论]高频振荡器用于大容量全肺灌洗治疗尘肺能提高灌洗效果,减少灌洗液残余量,加快术后恢复,缩短住院时间。 Objective:To probe into both the therapeutic effect and safety of applying high frequency generator for patients with pneumoconioses treated with high-capacity whole lung lavage.Method:A total of 125 pneumoconioses treated with unilateral stage high-capacity whole lung lavage were selected and divided into two groups randomly based on their admission order.All cases had accepted unilateral high-capacity whole lung lavage via double-chamber tracheal cannula under venous and inhalation combined general anesthesia.During operation,patients of control group were given repeated hand hacking by surgeon or visiting nurses on the lavage side of the chest.However,patients of test group were applied high frequency generator on the chest for oscillation as assistance during operation.Then the therapeutic effect of the two group cases were evaluated by lung function test and the improvement of clinical symptoms.Results:In terms of lung function and the improvement of clinical symptoms,there were statistical significant differences between the two groups and between before and after the lavage(P〈0.05).In terms of remanent fluids,lung function,and clinical symptoms improvement,there were statistical significant differences between the two group cases after the lavage(P〈0.05 or P〈0.01).The average hospitalization time of test group cases was 16.94 d±1.57 d and was 18.31 d±2.02 d in control group cases.There was statistical significant difference between the two groups(P〈0.01).Conclusion:High frequency generator applied for patients with pneumoconioses treated with high-capacity whole lung lavage high frequency generator for patients with pneumoconioses treated with high-capacity whole lung lavage could enhance their lavage effect,reduce the remanent fluids,accelerate their postoperative recovery,and shorten their hospital stay.
