机采血小板在血站无偿献血中造成冲红原因及影响因素分析 被引量:1

Analysis of influencing factors and reasons of red blood cell spill-over in thrombocytapheresis occurred in voluntary blood donation in blood stations

摘要 目的探讨分析机采血小板在血站无偿献血中造成冲红原因及影响因素。方法对1保存完整资料的1999年5月11日—2010年11月7日的无偿献血者618例中,符合国家健康体检标准。经血站采血员采血前严格询问,按规程选择标准程序。采用CS-3000P lus血细胞分离机进行机采,并排除机器故障及操作不当而出现冲红21例为冲红组;正常血小板采集597例作为对照组,进行①冲红血小板献血员及正常血小板献血员(对照组)单采前手指血血细胞计数结果;②预稀释血放置时间的影响;③不同水平血小板患者的样品两法结果;④测定杯的影响;⑤环境因素的影响;⑥采血方法的影响等比较;⑦抗凝剂选用。结果①冲红血小板献血员及正常血小板献血员(对照组)单采前手指血血细胞计数除RDW(P<0.05)外,HGB(g/L)、PLT(×109/L)、MPV(fl)、MCHC(g/L)、MCV(pg)、MCV(fl)、PDW(%)均无差异(Pall>0.05);②机采血分离血小板应在120min内完成;③机采法随血小板数量减少,两者差异愈大(P<0.05),故推测机采血造成冲红机率为高;④测定杯选用质量条件要高;⑤所处环境温度特别重要;⑥采血部位严禁用力挤压;⑦抗凝剂选用EDTA-K2。结论机采血小板冲红的主要原因与献血者频繁度、环境温度、采血方法、测定杯选用质量、抗凝剂选用密切正相关,分离血小板应在120min内完成。但机采血造成冲红机率为高。 Objective To explore the influencing factors and reasons of red blood cell spill-over in thrombocytapheresis occurred in voluntary blood donation in blood stations.Methods Totally 618 voluntary blood donors with complete clinical data between May 11th 1999 and November 7th 2010 were enrolled in this study.All of them met the national stipulation physical examination standard.Before blood collection,phlebotomists from blood stations inquired strictly about the donors' healthy condition,then followed a standard program according to procedures;21 red blood cell spill-over cases(Platelets were collected by a CS-3000Plus cell separator),excluded the machine fault and misoperation,were investigated in this study.The normal control group consisted of 597 voluntary blood donors with normal platelet collection.The study include ①the blood count of finger-prick blood before hemapheresis in both groups;②the effects of pre-diluted blood store time;③the results of two methods in donors with different levels of platelets;④the effects of test cups;the comparison of platelet count in the two groups;⑤the effects of environmental factors;⑥the comparison of the effects of different blood collection methods;⑦the anticoagulant selection.Results The blood count of finger-prick before hemapheresis in both groups had no difference(P0.05) in HGB(g/L)、PLT(×109/L)、MPV(fl)、MCHC(g/L)、MCV(pg)、MCV(fl)、PDW(%),except RDW(P0.05).The separation of platelets should be finished in 120 minutes.The difference between the two groups(P0.05) is more significant with the decrease of platelet counts.The result suggested that thrombocytapheresis show a high incidence of red blood cell spill-over.Choose the test cups with high quality.The environmental temperature is particularly important.It shall be strictly forbidden to squeeze hard on the blood collecting location.Select EDTA-K2 as the anticoagulant.Conclusions Red blood cell spill-over caused by thrombocytapheresis was positively related to the donors' frequent degrees,environmental temperature,blood collection methods,quality of test cups and anticoagulant selection.The separation of platelets should be finished in 120 minutes.However,thrombocytapheresis show a high incidence of red blood cell spill-over.
