摘要
目的 研究人发角蛋白人工肌腱 ( human- hair keratin artificial tendon,HHKAT)的组织相容性、抗原性及其在上肢肌腱修复术或功能重建术中的应用。方法 取 18只 Wistar大白鼠 ,将 HHKAT(实验组 )及相同重量的正常人发 (对照组 ) ,分别埋植至大鼠脊柱两侧皮下。术后 2、8和 12周取材 ,比较两种材料的组织学反应。临床应用 HHKAT14例 ,另选 2 5例为对照组。术前、术后分别取其外周血 ,测定免疫球蛋白 Ig G,Ig A,Ig M,补体 C3 ,T淋巴细胞亚群以及 T淋巴细胞转化 ,并对其结果进行比较。结果 实验结果证实 HHKAT的组织相容性较好 ,对组织的刺激性小于正常人发 ( P<0 .0 5 )。 14例术前、术后的免疫学指标无明显差异 ( P>0 .0 5 ) ,与对照组相比也无明显差异 ( P>0 .0 5 ) ;证实 HHKAT对人体无明显的抗原性。患者切口均一期愈合 ,术后随访 13~ 2 5个月 ,功能与病种及肌力评定 ,患肢功能恢复优良率达 92 .8%。结论
Objective To investigate the biocompatibility and antigenity of human hair keratin artificial tendon (HHKAT) and evaluate its application in repair of tendons of the upper extremities. Methods HHKAT and normal human hair were implanted in the subcutaneous tissue at the two sides of the spine in 18 Wistar rats. Samples were taken from both sides of the spine 2, 8 and 12 weeks postoperatively to observe and compare the local histological reaction. HHKAT were clinically used in 14 cases. 25 cases served as control. The blood sample was taken preoperatively and postoperatively to detect IgG, IgA, IgM, C3, T lymphocyte subgroup and T lymphocyte transformation. Results The local histological reaction induced by the artificial tendon was lower than normal human hair ( P < 0 05). The pre and postoperative immunological paramaters of patients who received human hair keratin artificial tendon transplantation had no statistically significant difference ( P > 0 05). There was no difference between patients of HHKAT group and control group ( P > 0 05). All the wounds healed primarily. The excellent and good rate of the function after 13 to 25 months’ follow up was 92 8 %. Conclusions HHKAT has no significant antigenity to the human body. It is an ideal substitute for tendon material at the moment.
