摘要
目的探讨大型听神经瘤术中面神经保护技术及方法。方法 21例听神经瘤全部采用枕下乙状窦后入路,在显微外科基础上采用神经电生理监测技术,术后采用House-Brackmann(HB)分级方法对面神经功能进行评价。结果肿瘤全切除18例,次全切除3例。面神经解剖保留17例,无死亡病例。术后出院时按House-Brackmann分级,面神经Ⅰ~Ⅱ级13例,Ⅲ~Ⅳ级6例,Ⅴ~Ⅵ级2例。结论熟练的显微外科技术、神经电生理监测的应用及面神经保护的术中技巧是面神经保护的关键。
Objective To investigate the large acoustic neuroma surgery of the facial nerve protection technology and methods. Methods All 21 patients with acoustic neuromas using the suboccipital retrosigmoid approach, based on the use of microsurgical nerve electrophysiological monitoring techniques, postoperative use of House - Brackmann ( HB ) facial nerve function grading method of evaluation. Results The tumor was totally removed in 18 cases, subtotal resection in 3 patients. Keep the facial nerve anatomy in 17 cases, no deaths. After discharge, according to House - Brackmann classification, facial nerve in 13 cases grade Ⅰ-Ⅱ, Ⅲ-Ⅳ grade in 6 cases, Ⅴ-Ⅵ 2 cases. Conclusion skilled microsurgical^techniques, the application of neurophysiological monitoring and protection of intraoperative facial nerve facial nerve protection techniques is the key.
