荧光定量PCR检测脂肪肝患者血浆循环DNA及其临床意义

摘要 目的检测脂肪肝患者血浆循环DNA(plasma circulatingDNA,PC-DNA)是否发生显著改变,探讨PC-DNA作为脂肪肝早期筛查标志物的可能性。方法从本院体检人群中选取脂肪肝患者和年龄与性别都与之匹配的健康对照人群作为研究对象,采集受试者的全血,分离血浆,提取PC-DNA,采用荧光定量PCR对看家基因—人生长激素基因(HGH)的DNA浓度进行定量以评价PC-DNA水平;另外,我们还检测了每份样本中谷丙转氨酶(ALT)、谷氨酰转移酶(GGT)及高密度脂蛋白(HDL)的浓度。最后,利用t检验和Spearman's correlation coefficients等分析方法对数据进行统计分析。结果在脂肪肝患者中PC-DNA的水平(Median=4.10)明显高于健康对照人群的PC-DNA水平(Median=3.47),P值等于0.0329;分析PC-DNA水平与ALT、GGT及HDL之间的相关性,发现PC-DNA与ALT及GGT正相关,而与HDL呈负相关。结论检测脂肪肝患者PC-DNA水平,并联合ALT、GGT及HDL等生化指标,可筛查脂肪肝。 Objective To detect whether the level of plasma circulating DNA (PC-DNA) is significantly altered in the patients with fatty liver versus healthy controls, and explore the possibility of PC-DNA to be used as the marker for early screening of fatty liver. Methods The patients with fatty liver and the sex-age-matched healthy controls were selected from the health examination survey in our hospital. PC-DNA was extracted from the plasma isolated from the whole blood for each subject. Quantification of the house gene, human growth hormone (HGH), were performed using fluorescence quantitative PCR to determine the concentration of PC-DNA. Additionally, we tested the concentration of glutamic pyruvic transaminase (ALT), glutamyltransferase (GGT) and high density lipoprotein (HDL). Statistic analysis was performed by using t-test and Spearman's correlation coefficients with SPSS-V 13.0. Results The relative concentration (Median = 4.10) of PC-DNA in patients with fatty liver significantly increased compared to that in controls (Median = 3.47) (P= 0.0329). The concentration of PC-DNA was positive correlation with ALT and GGT, yet was negative correlation with HDL. Conclusions Combination with the con- centration of PC-DNA, ALT and GGT may be the novel indicators for early screening of fatty liver.
