脂蛋白相关性磷脂酶A2、超敏C反应蛋白与脑梗死患者颈动脉粥样硬化斑块的关系 被引量:19

Relationship between lipoprotein-associated phospholipase A2,high-sensitive C-Reactive Protein level and carotid atherosclerosis in cerebral infarction

摘要 目的临床研究急性脑梗死患者血清脂蛋白相关性磷脂酶A2(Lp-PLA2)、超敏C反应蛋白(hs-CRP)的水平与颈动脉粥样硬化斑块的关系。方法选择急性脑梗死患者96例采用双抗体夹心酶联免疫吸附法测定两组的血清Lp-PLA2、hs-CRP,同时检测血脂水平,与同期健康体检者49例对照。急性脑梗死患者根据颈总动脉平均内中膜厚度(IMT)、斑块积分法、颈动脉狭窄程度进行分组比较Lp-PLA2与颈动脉粥样硬化斑块的关系。并对血清Lp-PLA2水平与脑梗死关系做回归分析。结果脑梗死患者组血清Lp-PLA2h、s-CRP水平明显高于正常对照组(P<0.01);按IMT、斑块积分、狭窄程度不同分组时,各组间Lp-PLA2浓度比较差异有统计学意义(P<0.01);hs-CRP水平在内膜正常组与内膜增厚组间比较差异无统计学意义(P>0.05),与斑块形成组比较差异有统计学意义(P<0.01);中度硬化组和重度硬化组间、中度狭窄组和重度狭窄组间hs-CRP水平差异无统计学意义(P>0.05),与轻度硬化组、轻度狭窄组比较差异有统计学意义(P<0.01);脑梗死与Lp-PLA2、hs-CRP呈正相关(P<0.05)。结论血清Lp-PLA2、hs-CRP水平升高可能是脑梗死重要的危险因素,hs-CRP水平与颈动脉粥样硬化程度无关,而Lp-PLA2水平随着颈动脉粥样硬化程度加重而升高,可能是直接参与动脉粥样硬化作用,导致脑梗死。 Objective To clinical research of relationship between plasma lipoprotein-associated phospholipase A2(Lp-PLA2),high-sensitive C-Reactive Protein(hs-CRP) level and carotid atherosclerosis in acute cerebral infarction.Methods 96 acute cerebral infarction cases were selected to determine Lp-PLA2,hs-CRP by euzymelinked immunosorbent assay,and the levels of blood-fat were measured in two group.Another 49 healthy adults served as normal controls.In acute cerebral infarction cases,based on the results of ultrasound of carotid artery,the patients were grouped to compare the relationship between Lp-PLA2,hs-CRP level and carotid atherosclerosis according to the average IMT,the plaque scoring,and the level of carotid artery stenosis Regression analysis was performed to level Lp-PLA2 and cerebral infarction.Results Plasma Lp-PLA2,hs-CRP level were significantly higher in the acute cerebral infarction group than in the control group(P0.01);According to the average IMT,the plaque scoring,and the level of carotid artery stenosis,the patients were grouped.There were significant difference in the levels of Lp-PLA2(P0.01);Between normal intima-media thickness group and thick intima-media thickness group,there were no statistically significant difference(P0.05),and plaque group,there were statistically significant difference(P0.01) in the level of hs-CRP.Between moderate carotid atherosclerosis and severe carotid atherosclerosis,between moderate carotid artery stenosis and severe carotid artery stenosis,there were no statistically significant difference(P0.05),and mild carotid group,mild carotid artery stenosis,there were statistically significant difference(P0.01) in the level of hs-CRP.Cerebral infarction correlated positively with Lp-PLA2,hs-CRP(P0.05).Conclusion The higher serum Lp-PLA2 and hs-CRP are possible important danger factors of cerebral infarction.In acute cerebral infarction,Hs-CRP level is not associated with carotid atherosclerosis,Lp-PLA2 level increased with the increasing the severer carotid atherosclerosis,may straight to participate in artherosclerosis,even induce to acute cerebral infarction.
