摘要
目的探讨超声血流向量成像(Vector flow mapping,VFM)技术研究正常人心动周期不同时相左室腔内血流状态的价值。方法选取健康志愿者48例,在常规超声心动图检查基础上,采集左室腔内血流向量图,应用VFM软件DSA-RS1分析不同时相左室内血流向量变化、涡流及基本流的特点。结果等容舒张期左室腔内无明显涡流,仅少量朝向心尖的低速血流;舒张早期和晚期以层流为主,二尖瓣下可见小涡流,圈数少,舒张中期为充满左室腔的大涡流,圈数多;等容收缩期以涡流为主;射血期以快速射入主动脉的层流为主,快速射血期二尖瓣下见圈数少的小涡流,缓慢射血期基本无涡流。左室腔内6个有涡流的时相中,涡流纵径、横径及圈数差异有统计学意义(P<0.05)。结论 VFM技术能显示不同时相左室腔内血液流态变化,可用于定量评价左室血流动力学改变。
Objective To discuss the value of using vector flow mapping(VFM) in normal left ventricular flow in different phases of cardiac circle.Methods Forty eight healthy controls were involved.After the routine echocardiography,the left ventricular vector diagram were acquired.The characteristic of flow vector,basic flow and vortex in left ventricular chamber during different phases of cardiac circle were analyzed by VFM DSA-RS1 software.Results No vortex except a little low blood flow towards the apical was found in isovolumic relaxation.The blood flow in early diastole and late diastole was mainly laminar flow.Small vortex rings of small number were found under the mitral valve in both phases.While the left ventricular cavity was filled with large and many laps eddy in middle diastole.The blood flow in isovolumic contraction was mainly vortex.The flow in ejection was mainly laminar flow quickly launched into the aorta.In rapid ejection,small vortex rings of small number were found under the mitral valve,while almost no vortex was found in slow ejection.The transverse diameter,vertical diameter and number of the vortices were significantly different in 6 eddy phases(P0.05).Conclusion VFM can show the blood flow changes of different phases in left ventricular chamber.and quantitatively evaluate left ventricular hemodynamics.
