前路减压植骨内固定在基层医院胸腰椎骨折伴脊髓损伤治疗中的应用 被引量:9

摘要 目的探索在基层医院对胸腰椎爆裂骨折伴脊髓损伤行前路减压,植骨内固定的治疗效果。方法36例胸腰椎骨折伴脊髓损伤行前路减压,Kaneda器械固定8例,Z-Plate钢板系统固定28例。结果随访6个月~4年,治疗后胸腰段脊柱生理曲度基本恢复,内固定无松动和断裂,植骨融合好,神经功能恢复(按Frankel分级)。总有效率为89%,完全恢复正常为25%。结论在基层医院,对胸腰椎骨折伴脊髓损伤行前路减压,植骨融合内固定治疗,取得较好的疗效和一定的社会和经济效益。 Objective To report an investigation on the curative effects of thoracolumbar spine fracture complicated with spinal cord injury treated by anterior decompression,bone grafting and internal fixation in basic hospital. Methods The 36 cases of thoracolumbar spine fracture patients with spinal cord injury were included and underwent anterior decompression, following 8 cases with Kaneda internal fixation and 17 cases with Z-plate steel plate internal fixa- tion. Results All cases got the follow-up for 6 months-4 years. The spinal deformity was corrected. The internal fixations were firm. The intervertebral bone grafting grew well. The recovered nervous function were evaluated with the criteria of Frankel. The overall effective rate was 89%,with 25% of complete recovery. Conclusion In basic hospital, the above-mentioned "anterior decompression,bone grafting and internal fixation"treatment for thoracolumar fracture with spinal cord injury has unique curative advantages, taking better society benefit and economy benefit.
