儿童非抽搐性癫痫发作与心因性非癫痫发作肌酶变化对比分析

Comparative Analysis on Variation of Cardiac Muscle Enzymes in Children with Non-convulsive Epilepsy or Psychogenic Non-epileptic Seizures

摘要 目的观察比较儿童非抽搐性癫痫发作与心因性非癫痫发作血清肌酶的变化,从而为两种疾病的鉴别诊断提供一定的理论依据。方法选取2006年1月-2010年6月期间我科80例住院患儿及50例同期健康儿童作为研究对象,其中80例患儿中,45例为非抽搐性癫痫发作,35例为心因性非癫痫发作,分别测定三组研究对象乳酸脱氢酶(LDH)、肌酸磷酸激酶(CK)、羟丁酸脱氢酶(HDBH)及肌酸磷酸激酶同工酶(CK-MB)血清浓度,并进行分析比较。结果非抽搐性癫痫发作患儿CK-MB血清含量为(14.21±5.02)U/L,对照组CK-MB血清含量为(10.87±3.58)U/L,前者显著高于后者,差异有统计学意义(P<0.05),两组儿童LDH、CK及HDBH血清水平差异无统计学意义;心因性非癫痫发作组与对照组比较,LDH、CK、HDBH及CK-MB血清含量差异无统计学意义。结论儿童非抽搐性癫痫发作时肌酸磷酸激酶同工酶(CK-MB)血清含量显著增高,心因性非癫痫发作肌酶改变不明显,监测肌酶血清含量不仅可作为判断癫痫发作的重要标志物,而且利于儿科医生将儿童非抽搐性癫痫发作与心因性发作进行区分,但由于CK-MB特异性不高,临床上仅供诊断参考。 Objective To observe and compare the variation of cardiac muscle enzymes between non-convulsive epilepsy and psychogenic non-epileptic seizures in children,and?to provide a theoretical basis for differential diagnosis of the two diseases.Methods Eighty children with epilepsy hospitalized in our hospital from January 2006 to June 2010 and 50 healthy children were served as the study objects.Among 80 children with epilepsy,45 were non-convulsive epilepsy and 35 were psychogenic non-epileptic seizures.The serum concentrations of Lactate dehydrogenase(LDH),creatine phosphokinase(CK),hydroxybutyrate dehydrogenase(HDBH),creatine kinase isoenzymes(CK-MB) in the three groups were detected and compared.Results The serum concentration of CK-MB in non-convulsive epilepsy group(14.21±5.02 U/L) was significantly higher than that in control group(10.87±3.58 U/L),and the difference was statistically significant(P0.05).No statistically significant difference was found in the serum concentrations of LDH,CK and HDBH between non-convulsive epilepsy group and control group.There was no statistically significant difference in the serum concentrations of LDH,CK,HDBH and CK-MB between psychogenic non-epileptic seizures group and control group.Conclusions The serum concentration of CK-MB in non-convulsive epilepsy children increases significantly,while the cardiac muscle enzymes in psychogenic non-epileptic seizures children show no significant change.The concentrations of cardiac muscle enzymes can not only act as the important epilepsy diagnosis markers,but also serve as the indexes to distinguish between non-convulsive epilepsy and psychogenic non-epileptic seizures.But CK-MB has a limitation,such as low specificity in clinical diagnosis.
