依那普利对慢性阻塞性肺疾病患者缓解期Ang-Ⅱ表达水平和粘液高分泌的影响 被引量:2

Effects of Enalapril on Serum Ang-Ⅱ Expression and Mucus Hypersecretion in Patients with COPD in Stable Stage

摘要 目的研究依那普利对COPD患者血清中血管紧张素Ⅱ(Angiotensin-ⅡAng-Ⅱ)表达水平及黏液高分泌的影响,初步探讨依那普利在COPD黏液高分泌中的治疗作用及机制.方法以合并原发性高血压(2级及2级以下)的COPD缓解期住院患者为研究对象,入选病例单盲随机分为常规治疗组(常规使用舒利迭、异丙托溴胺、氨茶碱、氨氯地平);依那普利治疗组(常规治疗的基础上加用依那普利10 mgqd).分别于患者出院前1 d,出院后第3周、第6周分别采取每名患者的静脉血,采用酶联免疫吸附法(ELISA)测定Ang-Ⅱ蛋白含量,同时计量患者痰量.结果依那普利治疗组与常规治疗组相比第3、6周血液中的Ang-Ⅱ明显降低(P<0.05),且依那普利治疗组痰量较常规治疗组减少(P<0.05).结论依那普利治疗能有效减少患者气道粘液高分泌,其作用机制可能与Ang-Ⅱ的下调有关. Objective To investigate the effects of enalapril on serum Ang-Ⅱ expression and mucus hypersecretion in patients with chronic obstructive pulmonary disease(COPD)in stable stage.And to detect the therapeutic effect and mechanism of enalapril in patients of COPD with mucus hypersecretion.Methods The COPD patients with primary hypertension(grade 2 and below)were taken as of study objects.Eligible patients were randomly divided into conventional treatment group(routine using Seretide,ipratropium bromide,ammonia theophylline,amlodipine)and enalapril treatment group(on the basis of conventional therapy plus enalapril 10 mg qd)by single-blind methods.The venous blood were taken for erery patient on the day before patients were discharged and 3 weeks,6 weeks respectively after discharge.Meanwhile,the amount of sputum from patients were collected at the same day and then determined sputum volume.The protein of Ang-Ⅱ was detected by using enzyme-linked immunosorbent assay(ELISA).Results The levels of Ang-Ⅱof patients in 3 and 6 weeks in enalapril group were significantly lower than conventional treatment group(P0.05).Compared withconventional treatment,the volume of sputum of patients in enalapril group was decreased significantly in 3 and 6 weeks(P0.05),indicating the Ang-Ⅱand sptum decreased after enalapril treatment.Conclusion Enalapril exhibites a marked effect on the reduction of sputum in COPD patients with airway hypersecretion,the underling mechanism may be associated with the downregulation of Ang-Ⅱ.
