乳腺良恶性病变细胞形态和核仁组织区嗜银蛋白定量的意义

摘要 目的:研究乳腺良恶性病变核仁组织区嗜银蛋白和细胞形态定量。方法:利用改良的AgNOR 染色和传统的HE染色测定乳腺良恶性病变AgNOR 颗粒面积、数目和细胞形态定量。结果:(1) 细胞面积、核面积、核质比值、细胞周长、AgNOR 颗粒面积及颗粒数目在乳腺良恶性病变之间存在显著差别( P< 0-0005) ;(2) 细胞面积、核面积及颗粒面积在Ⅰ级乳腺癌与Ⅱ级和Ⅲ级乳腺癌之间有显著差别( P< 0-01) ,核质比值在Ⅰ级与Ⅲ级之间有显著差别( P<0-01) ;(3) 颗粒数目在Ⅲ级乳腺癌与Ⅱ级、Ⅰ级乳腺癌有显著差别(P< 0-05 和P< 0-005) ;(4) 细胞形态各参数与颗粒面积和颗粒数目高度相关。结论:AgNOR 和细胞形态定量可作为乳腺良恶性病变鉴别、乳腺癌分级的客观指标。 Objective:To study nuclear organizer region-associated protein and cytomorphology in breast benign and malignant lesions. Methods: Improved AgNOR and traditional HE staining were used to measure granular number and area of AgNOR and cytomorphological parameters. Results: (1)There were highly significant discriminitions of cellular area,nuclear area,nucleus/cytoplasm,cellular perimeter and granular area and number between breast benign and malignant lesions(P<0 0005). (2)Comparing with grade Ⅰ,cellular area,nuclear area and granular area and number were higher in grade Ⅱ and Ⅲ. But nucleus/cytoplasm was higher in grade Ⅲ than grade Ⅰ. (3)There existed significant difference of granular number between grade Ⅲ and Ⅱ, grade Ⅲ and Ⅰ. (4) All cytomorphological parameters were correlated with granular number and area. Conclusion:Cytomorphological parameters and granular area and number can be taken for objective signs for discriminiting breast benign and malignant lesions and grading of breast carcinoma.
