摘要
目的探讨铁过负荷对2型糖尿病(T2DM)患者颈动脉粥样硬化的影响。方法对江西省T2DM患者根据颈动脉超声检查结果分为合并颈动脉硬化组(A组,60例)和不合并颈动脉硬化组(B组,60例),并将健康体检且无糖尿病者为对照组(C组,60例)。对所有入选者均行空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、空腹和餐后2 h C肽、血清三酰甘油(TG)、总胆固醇(TC)、血清铁蛋白(SF)、血清铁(SI)、总铁结合力(TIBC)、转铁蛋白饱和度(TS)等检测。结果与C组比较,A组与B组的SF、SI、TS均明显增高(P<0.01),TIBC则明显下降(P<0.01);A、B 2组间SF、SI、TIBC、TS差异均有统计学意义(P<0.05);铁过负荷在C、B、A 3组患者中依次升高,血糖、空腹和餐后2 h C肽3组之间差异有统计学意义(P<0.05或P<0.01)。结论江西省T2DM患者具有很高的铁负荷水平;铁过负荷与T2DM之间存在着相互影响,铁过量可能会影响到T2DM患者颈动脉粥样硬化及并发症的发生。
Objective To investigate the effect of iron overload on carotid artery atherosclerosis in patients with type 2 diabetes（T2DM）.Methods According to carotid ultrasound examination,T2DM patients were divided into carotid atherosclerosis group（group A）and non-atherosclerosis group（group B）,with 60 patients in each group.In addition,60 healthy control subjects without diabetes were recruited（group C）.Fasting plasma glucose（FPG）,2-hour postprandial plasma glucose（2h PG）,glycosylated hemoglobin（HbA1c）,fasting C peptide,and 2-hour postprandial C peptide,serum triglyceride（TG）,total cholesterol（TC）,serum ferritin（SF）,serum iron（SI）,total iron-binding capacity（TIBC）,and transferrin saturation（TS） were measured in all patients.Results Compared with group C,the levels of SF,SI and TS increased and TIBC decreased in group A and group B（P0.01）.The differences in SF,SI,TIBC and TS were significantly different between group A and group B（P0.05）.Compared with group B,iron overload increased in group A but decreased in group C.There were significant differences in blood glucose,fasting and 2-hour postprandial C peptides among the three groups（P0.05 or P0.01）.Conclusion T2DM patients in Jiangxi have high levels of iron overload,and there is an interaction between iron overload and T2DM.Iron overload may affect carotid atherosclerosis and its complications in patients with T2DM.
