经食管超声引导下介入封堵治疗房间隔缺损的应用研究 被引量:16

摘要 目的:探讨非X线监测下采用经食管超声(TEE),在内科介入封堵治疗房间隔缺损(ASD)中的应用价值。方法:继发孔型ASD患者31例,缺损大小10~30 mm。所有病例完全采用在TEE引导下,置入Amplatzer封堵器闭合ASD,监测封堵的全过程,并在封堵后即刻评价疗效。结果:31例患者在TEE监测引导下封堵器置入均获成功,术中即刻观察仅有3例患者封堵器边缘存在微量残余分流。结论:内科介入封堵治疗ASD时,若适应证选择恰当,在经验丰富操作熟练的术者实施下,可不依赖X线监测,将TEE作为唯一监测和引导手段完成封堵,且方法简便、效果可靠、费用低廉并减少了X线的损伤。 Objective:To assess the application value of transesophageal echocardiography in medical interventional closure of atrial septal defect(ASD) without x ray monitoring.Methods: Thirty one patients were involved in this study.The defect diameter was 10-30 mm.Each patient was positioned a Amplatzer device to close the defect in monitoring and guarding by TEE.To evaluate the therapeutic effect shortly after operation.Results:All patients were successfully positioned closure devices by TEE monitoring and guiding.Only 3 cases had micro amounts residual shunt instantly after operation.Conclusion: In medical interventional closure treatment of ASD,TEE may be used the only monitioring and guiding method for its convenient and reliable effect on the condition of applicable indication.
