单剂量布托啡诺联合吗啡用于剖宫产术后硬膜外镇痛的观察

摘要 目的观察单剂量布托啡诺联合吗啡对剖宫产产妇术后的镇痛效果及不良作用。方法选择在腰硬联合麻醉下行剖宫产产妇50例,ASAⅠ-Ⅱ级,均采用硬膜外单次镇痛,随机分为两组:布托啡诺组(B组,n=25),对照组(C组,n=25)。胎儿娩出后给予镇痛首剂。B组:镇痛首剂采用布托啡诺1mg+吗啡1.5mg+0.25%罗哌卡因共6ml,C组为吗啡2.5mg+0.25%罗哌卡因共6ml。观察术后24h内产妇视觉模拟评分(VAS)、舒适度评分(BCS)及皮肤瘙痒、恶心、呕吐等不良反应的发生率。结果①两组的VAS、BCS差异无显著性(P>0.05);②术后24h恶心呕吐发生率:B组(8%)与C组(36%)比较,明显减低(P<0.05);③术后24h皮肤瘙痒发生率:B组(24%)与C组(68%)比较,也明显减低(P<0.05)。结论单次布托啡诺联合吗啡用于剖宫产术后硬膜外镇痛,镇痛效果满意,降低术后恶心呕吐及皮肤瘙痒发生率。 Objective To observe the efficacy of analgesia and side effects of single dose butorphanol in post operative epidural analgesia with morphine after caesarean section.Methods 50 patients(ASAⅠ-Ⅱ) scheduled for single dose post operative epidural analgesia were divided into two groups randomly:Group B(Butorphanol,n = 25) and Group C(Control,n = 25).Loading dose was administered after the fetus out.Analgesia formula was as follows:①Group B,0.25% ropivacaine plus butorphanol 1 mg and morphine 1.5 mg.② Group C:0.25% ropivacaine plus morphine 2.5 mg.The VAS score,BCS score(Bruggrmann Comfort Scale),incidence rate of PONV(postoperative nausea and vomiting) and pruritus were observed in 24 hours post operatively.Results ①The VAS score and BCS score were similar in both groups.②The incidence rate of PONV in group B(8%) was significantly lower than that in group C(36%).③The incidence rate of pruritus in group B(24%) was significantly lower than that in group C(68%).Conclusion Single dose butorphanol in post operative epidural analgesia with morphine after caesarean section supply satisfactory analgesia with lower incidence rate of PONV and pruritus.
