摘要
目的探讨持续质量改进程序(continuous quality improvement,CQI)对减少输血前标本差错的效果。方法统计2007年7月~2011年3月我科收到的输血前血液标本169599份,其中2007年7月~2009年6月79115份(实施CQl前),2009年7月~2011年3月90484份(实施CQI后)。对所有标本实施两次ABO血型验证。自2009年7月起,针对错误标识和错误采集的标本采取CQI过程,比较实施CQI前后的错误标识标本和校正的错误采集标本比例。结果实施CQI后,错误标识标本比例从1/2.90增加到1/207,但随着时间进展而逐渐下降;校正的错误采集标本比例从1/1485降至1/4133,并维持在较低水平。结论实施CQI有助于改进标本采集过程,减少标本采集错误。
Objective To evaluate the effect of a continuous quality improvement (CQI) program in reducing the incidence of sample errors for pre-transfusion testing. Methods A total of 169599 blood samples for pre-transfusion testing were collected by this laboratory during July 2007 and March 2011, among which, 79115 samples were obtained from July 2007 to June 2009 (before CQI application) and 90484 samples were obtained from July 2009 to March 2011 (after CQI application). A double specimen policy for ABO type verification was implemented for all samples. The CQI process focusing on mislabeled samples and miscollected samples (wrong blood in tube, WBIT) has been implemented Since July, 2009. The mislabeled samples rate and corrected WBIT rate were compared before and after application of CQI. Results After the implementation of CQI, the mislabeled samples rate increased from 1/290 to 1/207, but gradually decreased as time passed by. The corrected WBIT rate decreased from 1/1485 to 1/3714, and maintained at a relatively lower level. Conclusion The CQI program can help to improve the sample collection process and reduce miscollected samples.
