星状神经节阻滞对急性脑梗塞患者血浆相关血管舒缩因子和氧自由基的影响 被引量:11

Effects of Stellate Ganglion Block on Related Vasomotor Factors and Oxygen Free Radicals of Plasma in Patients with Acute Cerebral Infarction

摘要 目的观察星状神经节阻滞(SGB)对急性脑梗塞患者内皮素(ET)、血浆降钙素基因相关肽(CGRP)及丙二醛(MDA)和超氧化物歧化酶(SOD)水平的影响,探讨SGB治疗急性脑梗塞患者的可能作用机制。方法急性脑梗塞患者(病程<3d)30例,随机分为SGB联合常规方法综合治疗组(SGB组)和常规治疗对照组两组,每组15例。SGB组在常规治疗的基础上加用SGB,以1%利多卡因10mL行SGB,1次/d,双侧交替,10次为一疗程。分别于治疗前(T1)、治疗后第5天(T2)、第10天(T3)取静脉血,采用放射免疫法测定血浆中ET、CGRP浓度,采用硫代巴比妥酸法及黄嘌呤氧化法测定血浆MDA含量和SOD活性。治疗前、治疗后20 d进行神经功能缺损(NFD)评分。结果与治疗前(T1)相比,SGB组和对照组治疗后(T2,T3)血浆ET浓度明显下降,ET/CGRP比值明显降低,血浆MDA含量明显降低,SOD活性明显升高(P<0.05);与对照组比较,SGB组治疗后T2、T3血浆ET浓度和MDA含量显著下降,CGRP浓度和SOD活性显著升高,ET/CGRP比值显著降低(P<0.05)。治疗后20 d神经功能缺损评分好转(P<0.05)。结论急性脑梗塞患者存在血浆ET/CGRP比例失衡,MDA含量升高,SOD活性降低;星状神经节阻滞联合常规疗法能调节血浆ET/CGRP比例失衡,减轻机体脂质过氧化反应,改善脑梗塞患者的神经功能,具有一定脑保护作用。 Objective To observe the effects of stellate ganglion block(SGB) on the concentration of endothelin(ET),calcitonin gene-related peptide(CGRP) and malondialdehyde(MDA),superoxide dismutase(SOD) in plasma in patients with acute cerebral infarction and to investigate the underlying mechanism of SGB in the treatment of patients with acute cerebral infarction.Methods Thirty patients with acute cerebral infarction(onset less than 3d) were randomly divided into two groups(n=15 for each) which were traditional treatment group(control group) and SGB+traditional treatment group(SGB group).SGB group received SGB on one side alternatively with 1% lidocaine 10 ml once a day for 10 days.Blood samples were taken the day before treatment(T1) and on the 5th and the 10th day after the treatment(T2,T3).Plasma ET and CGRP concentration were determined by radio-immunoassay.Plasma MDA concentration and SOD activity were detected in the way of Thiobarbituric Acid and Xanthine Oxidase.Neurological function deficit score was done for the patients before the treatment and 20 days after the treatment.ResultsCompared with T1,plasma ET concentration decreased significantly,the ratio of ET/CGRP decreased significantly,and plasma MDA concentration decreased significantly and plasma SOD activity increased significantly in both SGB group and control group after the treatment(P0.05).Compared with control group,plasma ET and MDA concentration decreased significantly,plasma CGRP concentration and plasma SOD activity increased significantly,and the ratio of ET/CGRP decreased significantly in the SGB group(P0.05) and its NFD score was better after the treatment(P0.05).Conclusion Generally,patients with acute cerebral infarction have the characteristics of unbalance ratio of ET/CGRP,increased MDA concentration and decreased SOD activity.SGB associated with traditional treatment can effectively regulate the ratio of ET/CGRP and inhibit lipid peroxidation and improve neurological functions of patients.alondialdehyde superoxide dismutase
