系统性红斑狼疮患者血清B淋巴细胞活化因子的检测及意义 被引量:4

Detection and significance of serum B cell activating factor in patients with systemic lupus erythematosus

摘要 目的探讨血清B淋巴细胞活化因子(BAFF)在系统性红斑狼疮(SLE)发病中的作用。方法采用ELISA法检测45例SLE患者(SLE组)血清BAFF水平,免疫比浊法检测血清补体C3、C4水平;计算疾病活动性(SLAM)和损伤指数(SLICC/ACR);对BAFF水平与补体C3和C4、SLAM、SLICC/ACR进行相关性分析。对照组为45例查体健康者。结果 SLE组血清BAFF水平明显高于对照组,抗dsDNA抗体阳性者(13例)BAFF水平明显高于阴性者,P均<0.001。BAFF水平与补体C3呈显著负相关(r=-0.298,P=0.047),与补体C4无明显相关性,与SLAM呈显著正相关(r=0.450,P=0.002);与SLICC/ACR呈显著正相关(r=0.388,P=0.009)。结论 BAFF可能参与了SLE的发病与发展,检测血清BAFF水平有助于判断SLE活动性及预后;应用BAFF拮抗剂有望改善SLE病情。 Objective To investigate the effect of serum B cell activating factor belonging to TNF family(BAFF) on the pathogenisis of systemic lupus erythematosus (SLE). Methods The serum BAFF levels of 45 SLE patients and normal controls were measured by ELISA, the C3 and C4 serum levels were detected quantitatively by turbidimentry. The SLE disease activity was assessed using the systemic lupus activity measure (SLAM) and organ damage using the systemic lupus in- ternational collaborating clinics/ACR (SLICC/ACR) damage index. The association of the serum BAFF level with serum C3 and C4 levels, SLAM, SLICC/ACR damage index were analyzed respectively. Results The serum BAFF level of SLE patients was significantly higher than that of normal controls. The serum BAFF level of anti-dsDNA antibody positive SLE patients was significantly higher than that of anti-dsDNA antibody negative patients, both P 〈 0.001. The serum BAFF level was negatively correlated with Ca level( r = - 0. 298, P = 0. 047 ), positively correlated with SLAM ( r = 0. 450, P = 0.002) and SLICC/ACR damage index (r = 0. 388, P = 0. 009 ). Conclusions BAFF is involved in the pathogenesis of SLE. The serum BAFF level may be an index for evaluation of SLE disease activity and prognosis; BAFF antagonists may improve the condition of SLE.
