摘要
目的探讨分析颈椎间盘突出在眩晕病中的发病机制,评价微创介入颈椎间盘消融术治疗椎间盘突出源性眩晕的疗效。方法我们将2009~2010年我科就诊的173例以眩晕为主诉的患者,按照随机、对照的原则进行分组,观察组行微创介入椎间盘消融,术后用抗生素、甘露醇、地塞米松、七叶皂苷钠静脉滴注治疗;对照组行颈椎牵引、推拿、手法正骨、小针刀松解结合药物静脉滴注治疗,采用视觉模拟评分(VAS)系统记录病人治疗前后以及随访时的症状情况进行疗效评价。结果观察组术后短期内即可达到较明显的症状改善,经长期随访,依然保持较好的临床效果,疗效评定上明显优于对照组,差异有统计学意义。结论颈椎间盘突出在眩晕的发病成因中占有重要地位,颈椎间盘消融术治疗因颈椎间盘突出导致的颈性眩晕具有较好的疗效。
Objective To explore the disease pathogenesis of cervical intervertebral disc herniation in vertigo and evaluate the therapeutic effect of minimal invasive cervical intervertebral disc ablation in the treatment of disc herniation vertigo.Methods 173 cases of patients with vertigo from 2009 to 2010 in our hospital were randomly divided into a treatment group and a control group.Patients in the treatment group underwent minimal invasive surgery by ablation of intervertebral disc combined with post-operative antibiotics,dexamethasone,and sodium aescinate intravenous dripping.In contrast,therapeutic means such as cervical traction,massage,bonesetting and small needle knife with loose solution combined with intravenous dripping were applied in the control group.The visual analog scale(VAS) system recorded the states of the patients before and after the treatment and in the follow-up period and assessment was made.Results The treatment group could achieve the immediate post-operative improvement in symptoms.After a long-term follow-up,this group could still manifest better clinical effect than the control group.The therapeutic assessment was superior to that of the control group.The two groups were statistically different.Conclusion Cervical intervertebral disc herniation accounts much for the pathogenesis of vertigo.Application of cervical intervertebral disc ablation for the treatment of cervical intervertebral disc herniation vertigo has a good therapeutic effect.
