摘要
目的探讨血清前列腺特异抗原(PSA)在前列腺癌诊断及其临床分期中的价值。方法采用放射免疫分析法检测前列腺癌患者(58例)和前列腺其他疾病患者(58例)血清PSA水平,对患者进行分组,比较前列腺癌组与前列腺其他疾病组的PSA水平,并将前列腺癌病人按分期分为A、B、C、D四组,统计各组在PSA正常值区(0~4ng/ml)、PSA值灰区组(4.1~10ng/ml)、PSA值升高区(10.1~20ng/ml)、高危区(>20ng/ml)四个区中的分布情况。结果前列腺癌组PSA为(72.34±69.87)ng/ml,对照组PSA为(3.47±2.37)ng/ml,两组比较,差异有统计学意义(t=2.68,P<0.05)。前列腺癌患者的分期和PSA值分布情况:A期7例,2例位于PSA灰区、3例位于PSA升高区、2例位于PSA高危值区;B期12例,2例位于PSA正常值区、1例位于PSA灰区、5例位于PSA升高区、4例位于PSA高危值区;C期9例、1例位于PSA灰区、3例位于PSA升高区,5例位于PSA高危值区;D期30例,2例位于PSA升高区、28例位于PSA高危值区。Spearman等级相关分析显示:PSA值与前列腺癌临床分期呈正相关(r=0.66,P<0.05)。结论 PSA在前列腺癌的诊断中有重要的价值;PSA值与前列腺癌临床分期呈正相关,PSA值越大,临床分期越晚,对前列腺癌患者的分期有重要的参考意义。可以提高预测特异度。
Objective To explore the application of serum prostate specific antigen(PSA) in diagnosis and clinical staging of prostate cancer.Methods The radioimmunoassay was used to test the PSA level of the 58 prostate cancer patients and 58 other prostate diseases patients;PSA levels of prostate cancer patients group and other prostate diseases patients group were compared and divided the prostate cancer patients to A,B,C,D four groups according to the clinical stage.The distribution of PSA levels of each group in four area such as normal area(PSA0-4 ng/ml),suspicious value area(PSA4.1-10 ng/ml),increased area(PSA10.1-20 ng/ml),high risk area(PSA20 ng/ml)was statistically analyzed.Results PSA value of prostate cancer patient group and control group were(72.34±69.87)ng/ml,(3.47±2.37)ng/ml,which had statistically significance(t=2.68,P〈0.05).The distribution of clinical stage and PSA values of prostate cancer patients:7 cases in stage A,2 cases among them were located in suspicious value area,3 cases in increased area,2 cases in high risk area;there were 12 cases in stage B and among them 2 cases were located in normal area,1 case in suspicious area,5 cases in increased area,4 cases in high risk area;there were 9 cases in stage C and among them 1 case were located in suspicious area,3 cases in increased area,5 cases in high risk area;there were 30 cases in stage D and among them,2 cases in increased area,28 cases in high risk area.Spearman rank correlation analysis showed that there was a positive correlation between PSA values and clinical stage of prostate cancer(r=0.66,P〈0.05).Conclusions Serum PSA has great value in diagnosis and clinical staging of prostate cancer.There was a positive correlation between PSA values and clinical stage of prostate cancer.The higher the PSA value,the clinical stage also identified as the more advanced stage.
