摘要
目的探讨64层螺旋CT多平面重组(MPR)和容积再现(VR)对肘关节损伤的临床应用价值。资料与方法应用64层螺旋CT对49例肘关节损伤患者进行各向同性的容积扫描,根据损伤部位及结构,在GE工作站(AW 4.3)上进行MPR和VR三维重组,并与轴位CT图像和X线平片进行对比。结果MPR结合VR图像能清晰显示肘关节骨折细节,共确诊骨折86处,累及关节面36例,诊断符合率分别为100%、100%,高于轴位CT图像(86%、77.8%)及X线平片(70.9%、69.4%),三种方法比较,差异有统计学意义(χ2=12.90,6.89、29.25,12.98,P值均<0.01)。结论64层螺旋CTMPR、VR图像可全面、准确显示肘关节骨折、脱位及关节面损伤情况,图像立体、直观,具有较高的临床应用价值。
Objective To evaluate the clinical value of multiplanar reconstruction(MPR) and volume rendering(VR) with 64-row helical CT in the injury of elbow joint.Materials and Methods Volume data of isotropic scanning for 49 cases with elbow joint injury was performed using 64-row helical CT,the MPR and VR images were reconstructed by GE workstation(AW4.3),the results were compared with the findings obtained from axial CT images(2DCT)and plain films.Results MMPR imaging combination with VR imaging could demonstrate the details of elbow fracture clearly,and 86 fractures were diagnosed,articular surface were involved in 36 cases.The coincident diagnosing rate were 100%、100%(86/86、36/36),which was higher than that by axial CT(86%,74/86、77.8%,28/36)and plain film(70.9%,61/86、69.4%,25/36),The differences between three modalities were significant(χ^2=12.90,6.89、29.25,12.98,P〈0.01).Conclusion MPR imaging combination with VR imaging of 64-row helical CT can comprehensively and accurately show the fracture,dislocation and articular surface of the injured elbow joint,which has great clinic value.
